目的探讨2型糖尿病患者行白内障囊外摘除术(extracapsu lar cataract extraction,ECCE)+后房型人工晶体(posterior chamber intraocu lar lens,PC-IOL)植入术后对泪膜的影响。方法选取行ECCE+PC-IOL植入术的2型糖尿病患者71例(80眼),分别在术前,术后3、7、14、30、90d行下睑中央泪河高度、泪膜破裂时间(breakup tim e,BUT)、基础泪液分泌试验(sch irm er’s-I test,S-I)、角膜荧光素染色检查。结果与术前比较,术后3、7、14d,BUT明显缩短,下睑中央泪河高度、角膜荧光素染色、S-I明显增高(P<0.01)。30d时S-I下睑中央泪河高度恢复至手术前水平,90d时BUT、角膜荧光素染色恢复至手术前水平,与术前比较差异无显著性(P>0.05)。结论ECCE+PC-IOL植入术可影响2型糖尿病患者泪膜的稳定性。
Objective To investigate the effect of extracapsular cataract extraction (ECCE) and posterior chamber intraocular lens (PC-IOL) implantation on the tear film in type 2 diabetic patients. Methods Totally 71 patients (80 eyes) with type 2 diabetes mellitus underwent ECCE + PC-IOL implantation were performed preoperative, 3, 7, 14, 30 and 90 days respectively. Breakup time (BUT), basal tear secretion test (SI) and corneal fluorescein staining. Results Compared with those before operation, the BUT significantly shortened at 3, 7 and 14 days after operation. The elevation of the tear in the lower eyelid, corneal fluorescein staining and S-I were significantly higher (P <0.01). At 30 days, the height of the tear in the middle of the lower eyelid of S-I was restored to the preoperative level. After 90 days, the BUT and corneal fluorescein staining returned to the preoperative level, with no significant difference (P> 0.05). Conclusion ECCE + PC-IOL implantation can affect the stability of tear film in type 2 diabetic patients.
