cifran是1990年印度Ranbaxy公司生产的人工合成的速效抗菌药,具有明显的广谱抗菌作用,能抑制革兰阳性和阴性菌(包括绿脓杆菌)生长.作者用cifran治疗30例病人,男16例,女14例,年龄24～82岁.其中破坏性(化脓、坏死、穿孔)阑尾炎并发局部性腹膜炎24例,破坏性胆囊炎并发周围脓肿6例.均手术切除阑尾或胆囊,并作腹腔引流.手术时静脉滴注cifran 200mg,以后每12h静滴200mg共2天.第3天改为口服,cifran 500mg每日2次,继续治疗5～9天,未用其他抗菌药.细菌学检查,98%的病例培养出革兰阴性菌,大部分对临床上常用抗生素如羧苄西林、氨苄西林、四环素、庆大霉素、氯霉素、阿米卡星和头抱噻吩等耐药,而对cifran均很敏感.阑尾炎病人,治疗2～3天,伤口分泌物细菌培养转阴.胆囊炎病人,治疗5～6天,分泌物培养转阴;2例菌血症患者治疗3天,血培养转阴.治疗前,病人全身情况严重,体温升高.血白细胞增多伴核左移;治疗3天后体温下降,5～8天血象恢夏正常.病人对cifran能很好地耐受,未见明显副作用和过敏反应.
