目的通过观察呼吸道感染患儿外周血超敏C-反应蛋白(hs CRP)、白细胞计数(WBC)及中性粒细胞比值(N%)变化,分析其在儿童呼吸道感染中的临床诊断价值。方法选取2016年10月-2017年3月该院儿科门诊确诊的呼吸道感染患者1 021例,按感染的病原体分为细菌感染组(321例)、病毒感染组(332例)和支原体感染组(304例),排除其他少见病原体感染及混合感染共64例。另以同期在该院门诊行健康体检的220例儿童作为对照组。比较分析各组外周血CRP、WBC及N%。结果细菌感染组hs CRP、WBC、N%与其他3组相比均明显升高(P<0.05),且以三个指标同时升高更为常见(51.7%);hs CRP升高伴WBC或N%升高(27.9%),hs CRP在细菌感染组的敏感性最高(90.0%);与其他3组比较,病毒感染组N%显著下降(P<0.05),WBC与hs CRP与对照组比较,差异无统计学意义(P>0.05);支原体感染组表现为hs CRP、WBC及N%三项中的一项或两项轻度升高。结论 hs CRP、WBC联合N%检测对于儿童呼吸道病原体感染的初步鉴别有重要意义,有助于在早期指导抗生素的应用,尤其hs CRP升高伴WBC和N%中一项或两项升高对于细菌感染的诊断价值极高。
Objective To observe the changes of hsCRP, WBC and neutrophil ratio (N%) in children with respiratory tract infection, and to analyze its clinical value in children with respiratory tract infection. Methods From October 2016 to March 2017, 1 021 cases of respiratory infections were diagnosed in the pediatric clinic of the hospital. According to the infected pathogens, they were divided into bacterial infection group (321 cases), virus infection group (332 cases) and mycoplasma infection group 304 cases), excluding other rare pathogens and mixed infections in 64 cases. The same period in the hospital outpatient health examination of 220 cases of children as a control group. Comparative analysis of peripheral blood CRP, WBC and N%. Conclusion The combination of hsCRP and WBC combined with N% detection is of great significance for the preliminary identification of respiratory tract pathogen infection in children and is helpful to guide the application of antibiotics in the early stage.
