目的探讨负压封闭引流(Vacuum sealing drainage,VSD)配合局部间断高浓度氧与单纯采用负压封闭引流治疗难愈性创面的效果。方法 2013年2月至2014年6月,本院收治需要进行负压封闭引流技术的难愈性创面患者50例,随机分成实验组和对照组,每组各25例。对照组给予负压封闭引流技术与按常规进行护理,连续24小时持续吸引。实验组在负压封闭引流技术的基础上每日两次向创面注入5L/min流量的氧气,期间暂停负压吸引,每次累计给氧15分钟;期间要保证避免VSD敷料贴膜渗漏。当停止使用负压封闭引流术后,约10天时间,观察并测量两组创面的愈合情况与最初相比较;计算出创面的愈合率与创面的平均愈合时间。结果实验组与对照组比较,10天时间的创面愈合率、创面植皮时间、创面愈合时间与植皮愈合率差异均有统计学意义(P<0.05)。结论负压封闭引流配合局部间断高浓度氧(每次15分钟每日2次)优于单独使用负压封闭引流技术治疗难愈性创面的愈合。
Objective To investigate the effect of vacuum sealing drainage (VSD) combined with intermittent high concentration of oxygen and the treatment of difficult-to-treat wound with negative pressure closed drainage. Methods From February 2013 to June 2014, 50 patients with refractory wounds undergoing negative pressure and closed drainage were randomly divided into experimental group and control group, with 25 cases in each group. The control group was given negative pressure closure drainage technology and routine care, continued to attract 24 hours. In the experimental group, 5L / min flow of oxygen was infused into the wound twice a day on the basis of negative pressure closed drainage technique. During this period, the negative pressure was suspended and accumulated for 15 minutes each time. During this period, leakage of VSD dressing foil should be avoided. After stopping the use of vacuum closed drainage, about 10 days time to observe and measure the healing of the two groups of wounds compared with the initial; calculate the wound healing rate and the average wound healing time. Results Compared with the control group, the wound healing rate, the wound healing time, the wound healing time and the healing rate of skin graft in 10 days were statistically significant (P <0.05). Conclusions Negative pressure closed drainage combined with local intermittent high concentration of oxygen (twice daily for 15 minutes) is better than the treatment of refractory wound healing with negative pressure closed drainage alone.
