目的 探讨应用乳腺常规和立体定位方法对临床隐匿性乳腺病变进行细针穿刺导丝定位活检术的应用价值。材料与方法 回顾 1997年 10月~ 2 0 0 0年 10月间对 39例临床未触及乳腺病变的患者采用细针定位活检术 ,其中常规定位法 2 7例 ,立体定位法 12例。结果 ( 1)病理结果 :39例中 ,发现早期乳腺癌 7例 (导管原位癌 2例 ) ,乳腺小叶增生 9例 ( 2例伴有非典型性增生 ) ,硬化性腺病 6例 ( 5例伴有上皮增生活跃 ) ,纤维腺瘤 12例 ,乳头状瘤 3例 ,浆细胞性乳腺炎 2例。 ( 2 )两种定位方法满意率比较 :立体定位 12例 ,全部一次定位成功 ,满意率为 10 0 % ;常规定位 2 5例一次定位成功 ,满意率为 93 %。两组数据经 χ2 检验 (P >0 .0 5 )。 ( 3)两种定位方法所用时间 :立体定位平均每人次为 41.6分钟 ;常规定位为 2 6 .2分钟。常规比立体定位节省时间平均每人次 15 .4分钟。结论 对临床不可触及、X线检查确有明显异常的乳腺病变 ,应积极采用细针定位后外科活检。常规定位法是一种简便、快捷和较准确的定位方法 ,可作为乳腺细针穿刺导丝定位的首选方法
Objective To explore the application value of conventional needle and stereotactic localization methods in the detection of clinical occult breast lesions by fine needle aspiration guided wire biopsy. Materials and Methods Retrospectively, fine needle biopsy was performed on 39 patients with clinically untouched breast lesions from October 1997 to October 2000. Of these, 27 were routinely located and 12 were stereotaxic. Results (1) Pathology: In 39 cases, 7 cases of early breast cancer (2 cases of ductal carcinoma in situ), 9 cases of breast lobular hyperplasia (2 cases with atypical hyperplasia) and 6 cases of sclerosing adenosis (5 cases) were found. With epithelial hyperplasia, fibroadenoma in 12 cases, papilloma in 3 cases, plasma cell mastitis in 2 cases. (2) Comparing the satisfaction rates of the two positioning methods: In 12 cases of stereotactic positioning, all successful positioning was successful with a satisfaction rate of 10%; 25 cases of routine positioning were successfully positioned with a satisfactory rate of 93%. Two sets of data were tested by χ2 (P > 0.05). (3) The time used for the two positioning methods: the average position per person for stereo positioning is 41.6 minutes; the conventional position is 26.2 minutes. The average time saved by stereo positioning is 15.4 minutes per person per time. Conclusions Clinically inaccessible and X-ray examinations do have significant abnormal breast lesions. Surgical biopsy should be performed after fine needle placement. The conventional positioning method is a simple, fast and accurate positioning method, which can be used as the preferred method for the positioning of the breast fine needle acupuncture guide wire.
