目的 :分析幽门螺杆菌(Hp)感染、焦虑抑郁与口腔扁平苔藓(OLP)发病及癌变的相关性。方法 :选取我院收治的OLP患者60例以及同期在我院进行体检的健康志愿者30例,分别设为OLP组和对照组。两组人员分别检测Hp、血清Hp-IgG抗体,CagA-IgG抗体和VacA-IgG抗体,采用医院焦虑抑郁量表(HADS)对两组人员进行焦虑抑郁测评。结果 :OLP组Hp强阳性和弱阳性表达数和对照组比较明显较高,差异有统计学意义。糜烂型OLP患者Hp阳性率和非糜烂型OLP患者比较,差异无统计学意义。OLP组中Hp-IgG抗体、CagA-HpIgG和VacAHpIgG抗体阳性率和对照组比较均明显较高,差异有统计学意义。糜烂型OLP患者Hp-IgG抗体、CagA-HpIgG和VacA-Hp IgG抗体阳性表达例数,差异无统计学意义。结论 :OLP与Hp感染及毒素CagA、VacA以及精神因素均具有一定相关性,进一步研究CagA、VacA这两种基因,对Hp感染引发的局部及全身疾病的治疗具有重要作用。此外,临床上还可通过对患者心理进行干预治疗,给予不同心理指导并配合适当药物,使治疗效果进一步提高。
Objective: To analyze the correlation between Helicobacter pylori (Hp) infection, anxiety and depression and the incidence of oral lichen planus (OLP) and its carcinogenesis. Methods: Sixty patients with OLP admitted to our hospital and 30 healthy volunteers who were in our hospital during the same period were selected as OLP group and control group. Hp, serum Hp-IgG, CagA-IgG and VacA-IgG antibodies were detected in two groups. An anxiety and depression test was conducted in both groups using the hospital anxiety and depression scale (HADS). Results: The positive and weak positive expression of Hp in OLP group was significantly higher than that in the control group, the difference was statistically significant. Erosive OLP patients with positive rate of Hp and non-erosive OLP patients, the difference was not statistically significant. The positive rates of Hp-IgG, CagA-HpIgG and VacAHpIgG in OLP group were significantly higher than those in control group, the difference was statistically significant. There was no significant difference in the positive expression of Hp-IgG, CagA-HpIgG and VacA-Hp IgG in patients with erosive OLP. CONCLUSION: There is a certain correlation between OLP and Hp infection and toxin CagA, VacA and mental factors. Further study of CagA and VacA genes plays an important role in the treatment of local and systemic diseases caused by Hp infection. In addition, the psychological treatment of patients can also be psychotherapy, giving different psychological guidance and with the appropriate drugs, the treatment effect to further improve.
