目的研究血脂成分与冠状动脉病变程度的相关性。方法对511例反复发作胸闷、胸痛,拟诊冠心病的住院患者行冠状动脉造影,使用Gensini积分评估冠状动脉病变程度,并与血脂成分及其他血生指标及危险因素进行统计分析。结果不同Gensini积分组间的HDL(P<0.001)、apoA(P=0.012)、TC/HDL(P=0.012)、LP(a)(P=0.006)、apoA/HDL(P=0.040)、apoB/HDL(P=0.004)、LDL/HDL(P=0.004)、TG/HDL(P=0.027)、NHDL/apoB(P=0.044)不同,差异有统计学意义;多元线性回归分析提示apoB/LDL-C(P<0.001)、肌钙蛋白T(P<0.001)和LDL-C/HDL-C(P=0.008)与冠脉病变严重程度关系密切。结论冠状动脉病变程度与异常血脂成分有关,apoB/LDL-C对冠心病的预测作用可能优于其他血脂指标及脂质成分组合。
Objective To study the correlation between the lipid composition and the severity of coronary artery disease. Methods 511 cases of recurrent chest tightness, chest pain, diagnosed coronary heart disease hospitalized patients underwent coronary angiography, the use of Gensini score assessment of coronary artery disease, and serum lipid composition and other blood metaplasia and risk factors for statistical analysis. Results HDL (P <0.001), apoA (P = 0.012), TC / HDL (P = 0.012), LP /HDL(P=0.004),LDL/HDL(P=0.004),TG/HDL(P=0.027),NHDL/apoB(P=0.044), the difference was statistically significant; multiple linear regression analysis suggested that apoB / LDL C (P <0.001), troponin T (P <0.001) and LDL-C / HDL-C (P = 0.008) were closely related to the severity of coronary artery disease. Conclusion The degree of coronary artery disease is related to abnormal lipid composition. The predictive value of apoB / LDL-C on coronary heart disease may be superior to other lipid parameters and lipid composition.
