目的了解山东省泰安市境内大汶河主干流域居民恶性肿瘤的死亡现状及其分布特征。方法采用横断面研究方法,对调查区及对照区居民2002-2004年恶性肿瘤死因进行回顾性调查。共调查辖区内6个县(市、区)44个乡镇;以国际疾病分类标准(ICD-10)进行各类肿瘤的死因编码和根本死因的推断,并计算死亡率、死因构成等指标。结果调查区恶性肿瘤年均标化率为189.17/10万,对照区年均标化率为162.63/10万,差异有统计学意义(u=5.81,P<0.001);年龄分布显示,45岁~死亡率随年龄增长而持续升高,60岁~组死亡率已达到较高水平,且不同年龄组癌症死亡谱各不相同,0~14岁和15~34岁年龄组以白血病为首位死因,35岁~各年龄组以食管癌为首位死因;性别分布显示,调查区和对照区男性标化死亡率分别为253.69/10万和225.54/10万,明显高于女性123.47/10万和100.49/10万(u=36.3,P<0.001;u=44.34,P<0.001)。结论大汶河主干流域恶性肿瘤死亡率较高,尤其是低年龄组的白血病和高年龄组的食管癌死亡率更高,且男性死亡率明显高于女性。
Objective To understand the status of death and the distribution characteristics of malignant tumors in residents of Dawenhe River in Tai’an City, Shandong Province. Methods A cross-sectional study was conducted to investigate the causes of death of malignant tumors in residents of the survey area and the control area from 2002 to 2004. A total of 44 towns and villages were surveyed in 6 counties (cities and districts) within the jurisdiction. In accordance with the International Classification of Diseases (ICD-10), the cause of death code and the cause of death of all kinds of tumors were inferred and the mortality and death causes were calculated. Results The average annualization rate of malignant tumors in the surveyed area was 189.17 / 100 000, with an average annual standardization rate of 162.63 / 100 000 in the control area, with significant difference (u = 5.81, P <0.001). The age distribution showed that 45 years ~ Mortality rate continues to rise with age, 60-year-old group mortality has reached a high level, and different age groups cancer death spectrum varies from 0 to 14 years and 15 to 34 age group leukemia as the first cause of death , 35-year-old to all age groups esophageal cancer as the leading cause of death; gender distribution showed that the male and female in the survey area and the standardized mortality were 253.69 / 100,000 and 225.54 / 100,000, significantly higher than the female 123.47 / 100000 and 100.49 / 100,000 (u = 36.3, P <0.001; u = 44.34, P <0.001). Conclusion The mortality rate of malignant tumors in Dawenhe River Basin is high, especially in the leukemia and high-age groups in the lower age group, and the male mortality rate is obviously higher than that in the female.
