目的:探讨高龄老年人群动脉弹性的影响因素。方法:选取高血压病高龄老年患者260例,分为单纯高血压组127例、高血压合并糖尿病组133例,并设同期查体或其他原因住院、不伴高血压及糖尿病的54例高龄老人为正常血压组,测定其身高、体重、血压及生化指标,用自动分析仪检测大动脉弹性指数(C1)和小动脉弹性指数(C2)。比较患者合并不同疾病时动脉弹性情况,对C1、C2进行相关及回归分析。结果:①同正常血压组相比,单纯高血压组的收缩压升高,C1和C2降低(P<0.05),高血压合并糖尿病组的体重指数、甘油三酯、空腹血糖、尿酸、收缩压升高,C1和C2降低(P<0.05);同单纯高血压组相比,高血压合并糖尿病组的体重指数、空腹血糖、尿酸升高,C2降低(P<0.05),而C1的差异无统计学意义(P=0.704)。②多元线性逐步回归分析显示,高龄老年患者的C1与年龄、收缩压、胆固醇负相关(R~2=0.507,P<0.05);C2与收缩压、年龄、低密度脂蛋白胆固醇、有无糖尿病负相关(R~2=0.464,P<0.05)。结论:年龄和收缩压与高龄老年患者动脉弹性密切关联,另外C1与胆固醇负相关,C2与低密度脂蛋白胆固醇及有无糖尿病负相关。
Objective: To investigate the influencing factors of arterial elasticity in the elderly population. Methods: Two hundred and seventy elderly patients with essential hypertension were enrolled in the study. One hundred and twenty-seven elderly patients with hypertension were divided into two groups. One hundred and twenty-seven patients with hypertension, 133 patients with hypertension and diabetes mellitus, 54 elderly patients without hospitalization for hypertension and diabetes mellitus The normal blood pressure group was measured for height, weight, blood pressure and biochemical indexes. The arterial elasticity index (C1) and arteriolar elasticity index (C2) were detected by automatic analyzer. The arterial elasticity of patients with different diseases was compared, and correlation and regression analysis of C1 and C2 were performed. Results: ① Compared with normotensive group, systolic blood pressure increased, C1 and C2 decreased (P <0.05), and body mass index, triglyceride, fasting blood glucose, uric acid, systolic blood pressure (P <0.05). The body mass index, fasting blood glucose, uric acid and C2 decreased in hypertensive patients with diabetes mellitus (P <0.05), while those in C1 Statistical significance (P = 0.704). (2) Multiple linear stepwise regression analysis showed that C1 was negatively correlated with age, systolic blood pressure and cholesterol in elderly patients (R ~ 2 = 0.507, P <0.05); C2 was related to systolic blood pressure, age, low density lipoprotein cholesterol, Negative correlation (R ~ 2 = 0.464, P <0.05). CONCLUSION: Age and systolic blood pressure are closely related to arterial elasticity in elderly elderly patients. In addition, C1 is negatively correlated with cholesterol and C2 is negatively correlated with low-density lipoprotein cholesterol and with or without diabetes.
