目的探讨并比较两种不同术式治疗臀肌挛缩的疗效。方法治疗臀肌挛缩症３５例，分别采用后外侧入路，切断变性的臀大肌腱膜、肌肉或作臀大肌肌腱“Ｚ”形延长及大粗隆后上方弧形小切口松解臀肌两种不同术式。结果前者创伤大、出血多、卧床时间长、恢复欠满意。后者具有创伤小、出血少、手术时间短、术后恢复快及可早期进行功能锻炼等优点。结论沿大粗隆后上方弧形小切口松解臀肌值得推荐 Objective To investigate and compare the therapeutic effect of two different surgical procedures on gluteal muscle contracture. Methods 35 cases of gluteal muscle contracture were treated by posterior lateral approach, the degenerative gluteus maximus aponeurosis, muscle or gluteus maximus tendon “Z” Two different surgical procedures. Results The former trauma, bleeding, bed time, recovery less satisfied. The latter has the advantages of less trauma, less bleeding, shorter operative time, faster postoperative recovery and early functional training. Conclusion It is recommendable to release the gluteus muscle with a small curved incision above the greater trochanter
