目的:探讨甲泼尼龙和冬眠合剂联合高压氧(HBO)治疗一氧化碳中毒后迟发性脑病(DEACMP)患者的疗效及其对血清S100B、GFAP表达的影响。方法:选取2017年4月至2019年7月山西白求恩医院急诊科收治的76例DEACMP患者作为研究对象,按照随机数字表法随机分为观察组与对照组,每组38例。对照组给予HBO治疗,观察组在对照组基础上给予甲泼尼龙、冬眠合剂治疗。比较2组患者的临床疗效、治疗前后的认知功能、细胞因子及血清中S100B、GFAP的水平变化。结果:治疗2~3个疗程后,观察组患者总有效率(92.1%)明显高于对照组(78.9%),差异有统计学意义(n χ2=4.547,n P=0.033)。治疗15 d后,观察组的蒙特利尔认知评估量表(MoCA)评分较治疗前显著升高(n P<0.05),在治疗30、60 d后,2组患者的Barthel指数评定量表(BI)和MoCA评分较治疗前均明显升高(n P<0.05),其中观察组治疗后30、60 d后BI和MoCA评分均明显高于对照组,差异均有统计学意义(n P<0.05)。治疗结束后,2组患者的IL-4、IL-10和IFN-β1水平较治疗前明显升高,其中观察组的各指标显著高于对照组(n P<0.05);IFN-γ水平较治疗前明显降低,其中观察组的IFN-γ水平显著低于对照组(n P<0.05)。2组患者血清中S100B、GFAP水平较治疗前明显降低,其中观察组治疗后S100B、GFAP水平显著低于对照组(n P<0.05)。n 结论:甲泼尼龙和冬眠合剂联合HBO治疗DEACMP患者具有较好的临床效果,值得临床推广应用。“,”Objective:To investigate the effects of methylprednisolone and hibernation mixture combined with hyperbaric oxygen (HBO) on patients with delayed encephalopathy after carbon monoxide poisoning(DEACMP) and the expressions of serum S100 calcium-binding protein B (S100B) and glial fibrillary acidic protein (GFAP).Methods:A total of 76 patients with DEACMP admitted to Emergency Department of Shanxi Bethune Hospital from April 2017 to July 2019 were selected as study objects and divided into the observation group and the control group according to the random number table, with 38 cases in each group. The control group was treated with HBO, and the observation group was treated with methylprednisolone and hibernation mixture on the basis of the treatment in the control group. The therapeutic effects, the changes of cognitive function, cytokines, and the levels of serum S100B and GFAP before and after treatment of the two groups were compared.Results:After 2 to 3 courses of treatment, the total effective rate in the observation group (92.1%) was significantly higher than that in the control group (78.9%), and the difference was statistically significant (n χ2=4.547, n P=0.033). On the 15n th day after treatment, the Montreal Cognitive Assessment (MoCA) score of the observation group was significantly increased (n P<0.05). On the 30n th and the 60n th days after treatment, the Barthel Index (BI) and MoCA scores of both groups increased (n P<0.05), and the BI and MoCA scores on the 30n th and the 60n th days after treatment in the observation group were significantly higher than those in the control group, with statistical significant differences (n P<0.05). The levels of interleukin-4 (IL-4), interleukin-10 (IL-10), and interferon-beta1 (IFN-β1) in both groups after treatment were significantly increased, and these cytokine indexes in the observation group were significantly higher than those in the control group (n P<0.05). The interferon-gamma (IFN-γ) level in both groups was significantly decreased, and the IFN-γ level in the observation group was significantly lower than that of the control group (n P<0.05). The levels of S100B and GFAP in both groups also significantly decreased as compared with those before treatment, and the levels in the observation group were significantly lower than those in the control group (n P<0.05).n Conclusion:Methylprednisolone and hibernation mixture combined with HBO can achieve a good therapeutic effect in treating DEACMP, which is worthy of clinical popularization.
