目的:探讨颅内动脉瘤的经颅多普勒(TCD)特点,为该病的临床诊断及疗效评估提供一种无创的辅助检查方法。方法:采用TCD检测38例颅内动脉瘤患者,观察颅内主要血管的各项血流参数、频谱形态、血流方向及动脉音频信号等,并与DSA检查进行对比分析。结果:实际检测35例,3例因声窗条件不佳,未检出信号。本组TCD总检出率为74%(26/35),>1cm的动脉瘤检出率为92%(22/24),提出动脉瘤样频谱的新概念。结论:TCD是颅内动脉瘤的一种可行性诊断方法,可作为行DSA检查前一种筛选颅内动脉瘤的辅助手段。也可作为治疗术后随访疗效的一种检查方法。
Objective: To investigate the transcranial Doppler (TCD) features of intracranial aneurysms and to provide a noninvasive auxiliary examination method for the clinical diagnosis and evaluation of the disease. Methods: Thirty-eight patients with intracranial aneurysm were examined by TCD, and various blood flow parameters, spectral patterns, blood flow direction and arterial audio signals were observed and compared with DSA. Results: The actual detection of 35 cases, 3 cases due to poor acoustic window conditions, no signal was detected. The overall detection rate of TCD in this group was 74% (26/35), and the detection rate of aneurysms> 92% (> 1cm) was higher than 1 cm. A new concept of aneurysm-like spectrum was proposed. Conclusion: TCD is a feasible method for the diagnosis of intracranial aneurysms. It can be used as an auxiliary method for screening intracranial aneurysms before DSA. Can also be used as a treatment for postoperative follow-up effect of a test method.
