目的探讨大龄高度近视散光患者准分子激光原位角膜磨镶术(LASIK)的治疗效果及其特点。方法对55例(95眼)大龄高度近视患者进行LASIK治疗,并对术前屈光度、屈光相关因素、预矫、实矫屈光度及手术后角膜曲率的改变、手术疗效进行统计学分析。结果 LASIK治疗的大龄高度近视患者中,高度近视角膜平均屈光力44.85 D,与低度近视角膜屈光力平均43.24D两者之间有较大差异,术后角膜屈光力为平均33.29 D;高度近视患者球镜屈光度-6.00~-9.75 D,共68眼占71.58%,超高度近视-10.00~-16.00 D,共27只眼占28.42%,两者轴镜屈光度-0.5~-3.75 D平均-2.76 D。治疗效果:术前裸眼视力0.01~0.1平均0.05;矫正视力0.5~1.2平均0.92。术后裸眼视力0.5~0.9共31只眼(32.63%);≥1.0共64只眼(67.37%),平均视力0.94。结论 LASIK治疗大龄高度近视散光患者,术前进行精确的屈光度、角膜地形图、角膜厚度、眼球轴长测量后加以综合分析,对预矫屈光度确定和预留屈光度设计、激光切削直径的选择是必要的,可以预防屈光回退,减少术后眩光,改善视觉质量。
Objective To investigate the therapeutic effect and characteristics of excimer laser in situ keratomileusis (LASIK) in elderly patients with high myopic astigmatism. Methods 55 cases (95 eyes) of elderly patients with high myopia were treated with LASIK. The preoperative refraction, refractive related factors, pre-correction, corrected refraction and postoperative corneal curvature were analyzed statistically. There were 31 eyes (32.63%) with uncorrected visual acuity (0.5 ~ 0.9), 64 eyes (67.37%) ≥ 1.0, and the average visual acuity was 0.94. Conclusion LASIK treatment of elderly patients with high myopia and astigmatism, preoperative precise refraction, corneal topography, corneal thickness, axial length of the eye after a comprehensive analysis of the determination of pre-refractive power and reserve diopter design, the choice of laser cutting diameter is necessary , Can prevent refractive regression, reduce postoperative glare, improve visual quality.
