目的:探讨大肠癌组织中Survivin基因及其主要剪接变构体与癌胚抗原(Carcinoembryonic antigen,CEA)CAM5mRNA表达的临床意义。方法:39例大肠癌切除标本,采集肿瘤组织、癌旁2.5cm组织和距离肿瘤10cm以上的正常大肠组织各1块,采用RT-PCR方法检测总survivin、survivin-△Ex3、survivin-2B、survivin-3B、survivin2α和CEACAM5 mRNA的表达。用凝胶图像分析系统或SYBRGreenⅠ实时荧光定量PCR检测上述基因mRNA的表达水平。结果:总survivin、survivin-△Ex3、survivin-2B、sur-vivin-3B、survivin2α和CEACAM5mRNA在大肠癌组织中的阳性表达率分别为100.0%、92.3%、92.3%、79.5%、87.2%和97.4%,均高于癌旁2.5cm组织中的38.5%、25.6%、28.2%、7.7%、12.8%和12.8%,差异均有统计学意义(P<0.01)。正常肠壁组织中上述mRNA的阳性表达率分别为17.9%、5.1%、7.7%、0.0%、2.6%和10.3%,与癌旁2.5cm组织比较,总survivin、survivin-△Ex3和sur-vivin-2B差异有统计学意义(P<0.05)。总survivin及其剪接变构体和CEACAM5mRNA的表达水平,大肠癌组织高于癌旁2.5cm组织(P<0.01);癌旁2.5cm组织与正常组织的差异均无统计学意义(P>0.05)。survivin剪接变构体mRNA的表达水平与一些临床病理参数有关(P<0.05)。结论:Survivin和其剪接变构体mRNA在大肠癌组织中呈高表达,表达水平与恶性临床病理参数有关。检测Survivin和其剪接变构体mRNA表达对大肠癌前预报与早期诊断比CEAmRNA更有价值。
Objective: To investigate the clinical significance of Survivin gene and its major splicing variants and carcinoembryonic antigen (CEA) CAM5 mRNA expression in colorectal cancer. Methods: Thirty-nine cases of colorectal cancer were resected, one tumor tissue, 2.5cm tissue adjacent to tumor and one normal colorectum more than 10cm were collected. The total survivin, survivin-Ex3, survivin-2B and survivin -3B, survivin2α and CEACAM5 mRNA expression. The expression levels of these genes were detected by gel imaging analysis system or SYBRGreen I real-time fluorescence quantitative PCR. Results: The positive rates of survivin, survivin-△ Ex3, survivin-2B, survivin-3B, survivin2α and CEACAM5 mRNA in colorectal cancer tissues were 100.0%, 92.3%, 92.3%, 79.5%, 87.2% and 97.4% %, Which were all higher than 38.5%, 25.6%, 28.2%, 7.7%, 12.8% and 12.8% of the 2.5cm adjacent tissues, the difference was statistically significant (P <0.01). The positive expression rates of the above mRNAs in normal intestinal wall tissues were 17.9%, 5.1%, 7.7%, 0.0%, 2.6% and 10.3% respectively. Compared with the adjacent tissue of 2.5cm, the total survivin, survivin- △ Ex3 and Sur-vivin -2B difference was statistically significant (P <0.05). The expression of total survivin, splicing variants and CEACAM5 mRNA in colorectal cancer tissues was significantly higher than that in adjacent tissues (2.5 cm, P <0.01). There were no significant differences in the expression of survivin, 2.5 cm and normal tissues (P> 0.05) . The expression of survivin splicing variant mRNA was correlated with some clinicopathological parameters (P <0.05). CONCLUSIONS: Survivin and its splice variant mRNA are overexpressed in colorectal cancer tissues. The expression level of Survivin is correlated with clinicopathological parameters. Detection of Survivin and its splice variant mRNA expression in colorectal cancer prognosis and early diagnosis of more valuable than CEAmRNA.
