通过5例眼球挫伤检查,以荧光血管造影为主分析了伤后眼底错杂多样的影像,证明唯有借助动静态造影方能在活体中探查出挫伤的发病机制,即血-视网膜屏障断裂所导致的组织结构改变,而非检眼镜所能视及。参照13例荧造影像,本文列举挫伤造成的视网膜和脉络膜的组织结构紊乱。以往认为只是单纯的视网膜水肿,实际上它早期已潜发神经上皮以及视弼膜和玻璃体界面的异常、视网膜出血、黄斑真假性穿孔等动力学的改变。通体荧光异常影像、时相改变以及色素上皮脱落到修整、重建可以得出与病检所见一致性损伤。血-视网膜屏障断裂则是损伤的核心。
