目的探讨 CD66c 及其基因 CEACAM6在成人急性白血病细胞上的表达及意义。方法体外培养 HL-60、K562、LCL721.221和 Jurkat 等急性白血病细胞,RT-PCR 检测急性白血病患者骨髓细胞和白血病细胞中 CEACAM6 mRNA 的表达,并用多参数流式细胞术检测白血病细胞及白血病患者骨髓细胞 CD66c 分子的表达。同时对199例白血病患者骨髓细胞作细胞遗传学分析,并对25例CD66c 阳性急性 B 淋巴细胞白血病(B-ALL)患者进行白血病微量残留病(MRD)分析。结果①HL-60、K562、LCL721.221和 Jurkat 细胞的细胞膜和细胞质 CD66c 均阴性。②127例急性髓系白血病(AML)患者中4例 CD66c 阳性(3.15%),以 M_2、M_4为主;79例 ALL 中 CD66c 阳性28例(35.44%),均为 B-ALL;CD66c 只在 common B-ALL(54例中有20例)和 pre B-ALL(11例中有8例)亚型表达,8例 PH~+B-ALL 患者 CD66c 均阳性。③MRD 阳性与阴性组患者比较,6个月内复发率差异有统计学意义(维持治疗14周时 MRD 阳性者8例,其中6例复发;MRD 阴性者17例,其中2例复发)。④CEACAM6 mRNA 在 CD66c 阳性 B-ALL 细胞强表达,在 HL-60细胞呈弱表达,在 K562、LCL721.221和 Jurkat 细胞不表达。结论 CD66c 在 ALL 中阳性表达是白血病 MRD 检测的标志之一。CD66c 抗原表达与 CEACAM6 mRNA 的表达高度相关。
Objective To investigate the expression and significance of CD66c and its gene CEACAM6 in adult acute leukemia cells. Methods HL-60, K562, LCL721.221 and Jurkat cells were cultured in vitro. The expression of CEACAM6 mRNA in bone marrow cells and leukemia cells was detected by RT-PCR. The leukemic cells and leukemia cells were detected by multi-parameter flow cytometry Expression of CD66c molecules in bone marrow cells. Meanwhile, 199 cases of leukemia patients with bone marrow cells were analyzed by cytogenetics and 25 cases of CD66c-positive acute lymphoblastic leukemia (B-ALL) were analyzed for MRD. Results ① The membrane and cytoplasmic CD66c of HL-60, K562, LCL721.221 and Jurkat cells were all negative. (4) CD66c positive (3.15%) were found in 127 cases of acute myeloid leukemia (AML), mainly M_2 and M_4; CD66c positive in 28 of 79 ALL cases (35.44%) were B-ALL; CD66c only in common The B-ALL (20 of 54) and pre B-ALL (8 of 11) subtypes were expressed, and all 8 CD + + B-ALL patients were positive for CD66c. ③MRD positive and negative group patients, the recurrence rate within 6 months was statistically significant (maintenance of 14 weeks MRD-positive in 8 cases, of which 6 cases of recurrence; MRD-negative in 17 cases, of which 2 cases of recurrence). ④CEACAM6 mRNA was strongly expressed in CD66c positive B-ALL cells, weakly expressed in HL-60 cells and not expressed in K562, LCL721.221 and Jurkat cells. Conclusion The positive expression of CD66c in ALL is one of the markers of MRD in leukemia. The expression of CD66c antigen is highly correlated with the expression of CEACAM6 mRNA.
