目的探讨正常产婴儿多次听力筛查未通过的原因及听力检查方法。方法对经听力筛查的初筛、42d复筛未通过的3月龄婴儿用瞬态耳声发射(TEOAE)和自动听性脑干反应(AABR)联合筛查并结合多种客观听力测试及纤维耳镜进行听力学综合评估。结果 62例(124耳)中38例(76耳)两项筛查均通过,占总人数的61.3%;11例(20耳)两项筛查均未通过,经听力诊断为不同程度的感音神经聋,占17.7%;13例(18耳)TEOAE未通过而AABR通过确诊为分泌性中耳炎,占20.9%。结论对多次听力筛查未通过正常产婴儿进行全面的听力评估,能早期发现听力损失,明确性质,避免漏诊、误诊。
Objective To investigate the causes of non-passing of multiple hearing screening in normal infants and methods of hearing test. Methods The first screening of the hearing screening, the 42-day re-screening of 3-month-old infants with transient otoacoustic emission (TEOAE) and automatic auditory brainstem response (AABR) screening and combined with a variety of objective hearing tests and Fiber otoscope for comprehensive audiological assessment. Results Of the 62 cases (124 ears), 38 cases (76 ears) passed both screening, accounting for 61.3% of the total; 11 cases (20 ears) failed to pass the two screening, the hearing was diagnosed to varying degrees 16.7% of the patients had neurosensory deafness, and 17.7% of them received acoustic hearing loss. Thirteen patients (18 ears) failed to pass TEOAE while AABR passed the diagnosis of secretory otitis media (20.9%). Conclusions Multiple audiometric screening can not make a comprehensive hearing assessment of normal infants, which can detect hearing loss early, clarify the nature, and avoid misdiagnosis.
