目的探讨卵巢癌组织淋巴管密度、功能形态及其在卵巢癌侵袭、转移中的意义。方法采用新型淋巴管内皮标志物D2-40对63例卵巢上皮性癌及30例卵巢良性上皮性肿瘤进行免疫组化染色,观察淋巴管形态结构,并计算淋巴管密度(LVD);以CD34标记微血管检测微血管密度(MVD)作为对比,并进行图像分析。分析卵巢癌中淋巴管密度及微血管密度与其分化程度、FIGO分期、腹膜种植、远处转移及淋巴结转移的关系。结果D2-40表达于淋巴管内皮细胞胞质;肿瘤不同区域的淋巴管密度、增殖活性和形态具有明显的异质性,在较大癌巢边缘区的淋巴管密度及增殖活性较卵巢良性上皮性肿瘤明显提高,且管腔多呈扩张状;癌巢边缘区LVD与卵巢癌患者年龄、瘤体大小及组织类型无关(均P>0.05),与卵巢癌分化程度、FIGO分期、腹膜种植、远处转移及淋巴结转移有关(P<0.05)。结论卵巢癌组织存在新生淋巴管,尤以癌组织边缘区明显,癌巢边缘区LVD检测可作为卵巢癌侵袭、转移等生物学行为及估计预后的指标。
Objective To investigate the lymphatic vessel density, functional morphology and its significance in the invasion and metastasis of ovarian cancer. Methods Immunohistochemical staining of 63 cases of ovarian epithelial carcinoma and 30 cases of benign ovarian epithelial tumor was performed with a new type of lymphatic endothelial marker D2-40. Morphology and structure of lymphatic vessels were observed and lymphatic vessel density (LVD) was calculated. CD34 Microvessel microvessel density (MVD) as a comparison, and image analysis. Analysis of ovarian cancer in lymphatic vessel density and microvessel density and the degree of differentiation, FIGO staging, peritoneal implantation, distant metastasis and lymph node metastasis. Results D2-40 was expressed in the cytoplasm of lymphatic endothelial cells. Lymphatic vessel density, proliferation activity and morphology in different regions of the tumor showed obvious heterogeneity. Lymphatic vessel density and proliferative activity in the marginal zone of larger cancer nests were significantly higher than those in benign ovarian epithelium LVD was not related with the age, tumor size and type of tissue (all P> 0.05), but not with ovarian cancer differentiation, FIGO stage, peritoneal implantation, Distant metastasis and lymph node metastasis (P <0.05). Conclusion There are lymphatic vessels in newly diagnosed ovarian cancer tissues, especially in marginal zone of cancerous tissue. The LVD detection in marginal zone of ovarian cancer can be used as an indicator of biological behavior such as invasion and metastasis of ovarian cancer and prognosis.
