目的探究不同的限食疗法对青少年肥胖2型糖尿病的治疗作用。方法 97例青少年肥胖2型糖尿病患者,随机分为极低热量限食组(31例)、低热量限食组(33例)和常规饮食组(33例)。对比三组体质量指数(BMI)、腰围及糖脂代谢情况。结果治疗前三组患者BMI、腰围、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)比较差异无统计学意义(P>0.05);三组患者治疗8、24周后BMI、腰围较治疗前显著降低(P<0.05);极低热量限食组、低热量限食组、常规饮食组在8、24周后TC、TG、LDL-C较治疗前显著降低(P<0.05);极低热量限食组24周后BMI、腰围、TC、TG、LDL-C均显著低于低热量限食组与常规饮食组(P<0.05);三组患者治疗前空腹血糖水平(FPG)、餐后2小时血糖(PPG)、糖化血红蛋白(Hb A1c)、HOMA-IR指数等比较,差异无统计学意义(P>0.05);三组患者治疗8、24周后FPG、PPG、Hb A1c、HOMA-IR等较治疗前显著降低(P<0.05);极低热量限食组治疗24周后FPG、PPG、Hb A1c、HOMA-IR等均显著低于低热量限食组与常规饮食组(P<0.05)。结论青少年肥胖2型糖尿病患者采用限食疗法,可有效降低体质量、腰围,并有效改善糖脂代谢情况,疗效确切。
Objective To explore the therapeutic effects of different diet-limited treatments on obese type 2 diabetes in adolescents. Methods A total of 97 adolescent patients with type 2 diabetes mellitus (T2DM) were randomly divided into minimal low calorie restriction group (31 cases), low caloric restriction group (33 cases) and conventional diet group (33 cases). Body mass index (BMI), waist circumference and glucose and lipid metabolism were compared between the three groups. Results There was no significant difference in BMI, waist circumference, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and triglyceride (TG) between the three groups before treatment (P <0.05). After 24 weeks, the BMI and waist circumference were significantly lower than those before treatment (P <0.05). TC, TG and LDL-C were lower in the diet with low calorie restriction group, (P <0.05). The BMI, waist circumference, TC, TG and LDL-C of the low-calorie restriction group were significantly lower than those of the low-calorie restriction group and the conventional diet group after 24 weeks The levels of FPG, PPG, HbAc and HOMA-IR before treatment were not significantly different (P> 0.05). The treatment of three groups was 8,24 FPG, PPG, HbA1c, HOMA-IR of FPG were significantly lower than those before treatment (P <0.05); FPG, PPG, HbA1c, Calorie restriction group and conventional diet group (P <0.05). Conclusion Adolescents with type 2 diabetes mellitus with limited diet therapy, can effectively reduce body weight, waist circumference, and effectively improve glucose and lipid metabolism, the exact effect.
