目的研究小儿先天性肾积水血、尿β2-微球蛋白(β2-microglobulin,β2-MG)测定对评价肾功能损害的意义。方法通过放射免疫法测定先天性肾积水患儿和正常儿童血、尿β2-MG的水平,比较观察组与对照组之间血、尿β2-MG水平差异。结果轻度肾积水患儿尿β2-MG含量较对照组明显升高,但血清β2-MG水平与对照组比较无显著性差异,中、重度肾积水患儿血、尿β2-MG含量与对照组比较明显升高。不同程度肾积水患儿间血、尿β2-MG含量比较有显著性差异。结论尿β2-MG是反映肾小管功能的灵敏指标,轻度先天性肾积水患儿尿β2-MG含量明显升高,提示轻度肾积水患儿已存在肾小管功能损害,血β2-MG含量较对照组无明显升高提示肾小球功能尚在正常范围,轻、中、重度肾积水患儿血、尿β2-MG含量有显著差异,提示不同程度肾积水肾功能损害程度不同,本研究证明,血、尿β2-MG水平对反映小儿先天性肾积水造成的肾功能损害具有较高的敏感性。
Objective To investigate the significance of congenital hydronephrosis and β2-microglobulin (β2-MG) in children with renal damage. Methods The level of blood and urine β2-MG in children with congenital hydronephrosis and normal children were determined by radioimmunoassay. The differences of blood and urine β2-MG levels between the observation group and the control group were compared. Results The urinary β2-MG level in children with mild hydronephrosis was significantly higher than that in the control group, but the level of serum β2-MG was not significantly different from that in the control group. Serum and urinary β2-MG levels in children with moderate and severe hydronephrosis Compared with the control group was significantly higher. The levels of blood and urine β2-MG in children with different degrees of hydronephrosis were significantly different. Conclusion Urinary β2-MG is a sensitive indicator of renal tubular function. The urinary β2-MG content in children with mild congenital hydronephrosis was significantly increased, suggesting that renal dysfunction, β2- MG content was significantly higher than the control group prompted glomerular function is still in the normal range, mild, moderate and severe hydronephrosis children, blood and urine β2-MG content was significantly different, suggesting that varying degrees of hydronephrosis renal dysfunction Different, this study proves that blood, urine β2-MG levels to reflect the children with congenital hydronephrosis caused by renal dysfunction has a higher sensitivity.
