目的回顾近10年成都战区部分官兵幽门螺杆菌(Hp)感染特点及其与上消化道疾病的关系,以指导Hp相关胃肠疾病的诊治。方法收集2005年1月~2015年1月在我院就诊的成都战区军人Hp感染患者病例资料,分析Hp感染特点及与上消化道疾病的关系。结果共纳入1248例因上消化道症状、需做胃镜检查患者,其中男性1104例,女性144例;年龄16~88(34.1±15.3)岁。男性Hp阳性者564例,感染率为51.1%;女性Hp阳性者85例,感染率为59.0%,男女Hp感染率无显著差异(P>0.05)。内镜下诊断:慢性胃炎(包括慢性非萎缩性胃炎及慢性萎缩性胃炎)962例,Hp阳性者483例。其中慢性非萎缩性胃炎伴糜烂143例,Hp阳性者79例;慢性萎缩性胃炎32例,Hp阳性者12例;消化性溃疡251例,Hp阳性者173例(68.9%),Hp阴性者78例,两者有显著差异(P<0.01)。病理检查:胃癌2例,Hp阳性者0例;没有不典型增生;肠化3例,Hp阳性者1例,Hp阴性者2例。结论本战区军人Hp感染率高(52.0%),影响官兵健康,应引起高度重视。Hp感染与消化性溃疡密切相关。
Objective To review the characteristics of Helicobacter pylori (Hp) infection and the relationship with Helicobacter pylori infection in some soldiers and soldiers in Chengdu Theater in recent 10 years to guide the diagnosis and treatment of Hp-related gastrointestinal diseases. Methods The data of Hp infected patients in military theater in Chengdu from January 2005 to January 2015 were collected to analyze the characteristics of Hp infection and its relationship with upper gastrointestinal diseases. Results A total of 1248 patients with upper gastrointestinal symptoms were enrolled, including 1104 males and 144 females, aged 16-88 years (34.1 ± 15.3 years). Among 564 male Hp positive patients, the infection rate was 51.1%. There were 85 female Hp positive patients with the infection rate of 59.0%. There was no significant difference in the Hp infection rate between men and women (P> 0.05). Endoscopic diagnosis: chronic gastritis (including chronic non-atrophic gastritis and chronic atrophic gastritis) 962 cases, Hp-positive in 483 cases. There were 143 cases of chronic non-atrophic gastritis with erosion, 79 cases of Hp positive, 32 cases of chronic atrophic gastritis, 12 cases of Hp positive, 251 cases of peptic ulcer, 173 cases (68.9%) of Hp positive, 78 For example, there is a significant difference between the two (P <0.01). Pathological examination: 2 cases of gastric cancer, Hp-positive in 0 cases; no atypical hyperplasia; intestinal metaplasia in 3 cases, Hp-positive in 1 case, Hp-negative in 2 cases. Conclusion The prevalence of Hp infection in servicemen in the theater is high (52.0%), which affects the health of officers and soldiers and should be given high priority. Hp infection and peptic ulcer are closely related.
