目的探讨甲胎蛋白(AFP)、糖链抗原19-9(CA19-9)和糖链抗原125(CA125)联合检测对早期肝癌诊断准确性的影响。方法选取我院115例肝病患者,包括早期肝癌患者55例和肝硬化患者60例,并选取同期健康体检者50例作为对照,分别检测三组的血清AFP、CA19-9和CA125水平。结果早期肝癌组和肝硬化组的AFP、CA19-9和CA125水平均显著高于正常对照组,早期肝癌组的AFP、CA19-9和CA125水平明显高于肝硬化组,差异有统计学意义(P<0.05)。ROC曲线分析显示:AFP以25μg/L为临界值,CA19-9以37 U/m L为临界值,CA125以235 U/m L为临界值,三者单独及联合鉴别诊断早期肝癌与肝硬化的敏感度、特异度和准确度比较为:三者联合>AFP>CA19-9>CA125,有显著性差异(P<0.05)。结论血清AFP、CA19-9和CA125联合检测有利于提高早期肝癌和肝硬化患者诊断的阳性率,对鉴别诊断具有重要的临床价值。
Objective To investigate the effect of combined detection of AFP, CA19-9 and CA125 on the diagnostic accuracy of early stage liver cancer. Methods A total of 115 patients with liver disease, including 55 patients with early-stage liver cancer and 60 patients with cirrhosis, were enrolled in this study. 50 healthy people were enrolled in this study. Serum levels of AFP, CA19-9 and CA125 were detected in three groups. Results The levels of AFP, CA19-9 and CA125 in early liver cancer group and cirrhosis group were significantly higher than those in normal control group. The levels of AFP, CA19-9 and CA125 in early liver cancer group were significantly higher than those in cirrhosis group (P < P <0.05). ROC curve analysis showed that: AFP 25μg / L as the critical value, CA19-9 37 U / m L as the critical value, CA125 as 235 U / m L as the critical value, the three separate and combined differential diagnosis of early liver cancer and cirrhosis The sensitivity, specificity and accuracy of the three combinations: AFP> CA19-9> CA125, there was a significant difference (P <0.05). Conclusion Combined detection of serum AFP, CA19-9 and CA125 is helpful to improve the positive rate of diagnosis in patients with early liver cancer and cirrhosis, and has important clinical value in differential diagnosis.
