目的:探讨温心汤对冠心病缓慢性心律失常患者心脏自主神经功能的影响.方法:将152例冠心病缓慢性心律失常患者随机分为对照组与治疗组,每组76例.对照组采用卡托普利、单硝酸异山梨酯、肠溶阿司匹林治疗,治疗组采用温心汤治疗.比较两组治疗前后临床症状、动态心电图平均心率、最慢心率、即刻心率及心率变异性指标[全部窦性心搏NN间期(standard deviation of NN intervals,SDNN)、每5 min NN间期平均值标准差(standard deviation of average 5 min NN intervals,SDANN)、相邻NN间期差值的均方根(square root of the mean squared differences NN intervals,RMSSD)、低频与高频比值(low frequency/jigh frequency,LF/HF)]等变化情况,并对两组临床疗效及安全性进行评价.结果:治疗组总有效率为92.11%,对照组总有效率为73.68%,治疗组优于对照组,差异有统计学意义(P<0.05);与治疗前比较,治疗后两组中医证候积分、SDANN、RMSSD均下降,而平均心率、最慢心率、即刻心率、SDNN、LF/HF均上升,差异均有统计学意义(P<0.05);治疗后两组比较,治疗组中医证候积分、SDANN、RMSSD均低于对照组,心率、最慢心率、即刻心率、SDNN、LF/HF均高于对照组,差异均有统计学意义(P<0.05);两组在治疗期间均未见肝功能、肾功能异常及不良反应.结论:采用温心汤治疗冠心病缓慢性心律失常具有较好临床疗效,能改善心脏自主神经功能.“,”Objective: To investigate the effect of Wenxin decoction on cardiac autonomic nervous function in coronary heart disease (CHD) patients with tardy arrhythmia. Methods: A total of 152 CHD patients with tardy arrhythmia were divided into the control group and the treatment group by random number table, with 76 cases in each group. The control group took captopril, isosorbide mononitrate and enteric coated aspirin, while the treatment group took Wenxin decoction. The changes of clinical symptoms, average heart rate of dynamic electrocardiogram, minimum heart rate (MiHR), instant heart rate and heart rate variability (HRV) indexes [standard deviation of NN intervals (SDNN), standard deviation of average 5min NN intervals (SDANN), square root of the mean squared differences NN intervals (RMSSD), low frequency/high frequency (LF/HF)] of two groups before and after treatment were compared, and clinical efficacy and safety of two groups were evaluated. Results: The total effective rate of the treatment group was 92.11%, which was higher than 73.68% of the control group (P<0.05). Compared with pre-treatment, the TCM symptom scores, SDANN and RMSSD of two groups decreased after treatment, but average heart rate, MiHR, instant heart rate, SDNN and LF/HF increased, and above indexes of the treatment group were better than the control group (P<0.05). There was no abnormal liver function, renal function and adverse reactions in two groups during treatment. Conclusion: Wenxin decoction for CHD patients with tardy arrhythmia can significantly increase clinical efficacy, relieve clinical symptoms, improve cardiac autonomic nervous function and promote body recovery, which has significance of clinical promotion.
