目的: 提高小脑蚓部各肿瘤CT 诊断的正确性。材料与方法: 95 例经手术、病理证实的小脑蚓部肿瘤,男性59 例,女性36例,术前均行CT 平扫及增强扫描。结果: 成神经管细胞瘤41 例( 男∶女= 21∶20 ,平均年龄17 .4 岁) ,CT 平扫以略高、等或略低混杂密度为主,增强后肿瘤可均匀或不均匀强化。星形胶质瘤29 例( 男∶女= 19∶10 ,平均20 .8 岁) ,CT 平扫以略低、低混杂密度为主,增强后可轻度、不规则强化。室管膜瘤10 例( 男∶女= 8∶2 ,平均23 岁) ,平扫以等、混合密度为主,增强多有均匀强化,常伴脑积水。成血管细胞瘤9 例( 男∶女= 5∶4 ,平均40 岁) ,平扫可分实质性和囊变两大类,增强后实质性肿瘤呈明显均匀强化,囊变者为大囊及附壁明显强化的小结节。另外少枝胶质瘤1 例,转移瘤3 例,脉络丛乳头状瘤1 例,海绵状血管瘤1 例。结论: 根据患者的年龄及CT表现有助于小脑蚓部肿瘤的鉴别诊断。
Objective: To improve the correctness of CT diagnosis of cerebellar vermis. Materials and Methods: A total of 95 cases of cerebellar vermis tumors were confirmed by operation and pathology. There were 59 males and 36 females. CT scan and enhanced scan were performed preoperatively. Results: There were 41 cases of medulloblastoma (male: female = 21:20, mean age 17.4 years). The CT scan was slightly higher, with equal or slightly lower mixed density, and the tumors could be even or uneven after enhancement strengthen. Twenty - nine cases of astrocytoma (M: F = 19:10, average 20.8 years old) had CT scan with slightly lower and lower miscellaneous densities, and enhanced mild and irregular enhancement. Ependymoma in 10 cases (male: female = 8: 2, mean 23 years), plain scan and so on, the main mixed density, enhancement and more uniform enhancement, often accompanied by hydrocephalus. Hemangioblastoma in 9 cases (male: female = 5: 4, average 40 years), plain can be divided into two categories of substantive and cystic change, enhanced solid tumors were significantly enhanced, cystic change were large capsule and Coat significantly enhanced nodules. Another small branch of glioma in 1 case, 3 cases of metastases, choroid plexus papilloma in 1 case, cavernous hemangioma in 1 case. Conclusion: According to the patient’s age and CT findings, it is helpful for the differential diagnosis of cerebellar vermis tumors.
