目的:了解东莞市围产儿先天性心脏病(先心病)的现状,为进一步制定先心病干预措施提供依据。方法:对2008年1月~2011年12月在东莞市各镇区住院分娩的孕满28周至产后7天的围产儿556 282例进行出生缺陷监测,并对监测所得的先心病资料进行分析。结果:①4年间东莞市医院监测围产儿先心病1 702例,平均发生率为3.06‰。②先心病的发生具有以下特征:孕母年龄30~35岁的先心病发生率明显高于其他各年龄组,差异有统计学意义(P<0.01);双胎(或多胎)妊娠先心病发生率高于单胎妊娠,差异有统计学意义(P<0.01);东莞市户籍孕母先心病发生率高于非东莞市户籍孕母,差异有统计学意义(P<0.01);男性先心病发生率高于女性,差异有统计学意义(P<0.01);③先心病合并其他畸形127例,占先心病的7.46%;④单一类型的先心病前3位顺位依次为:卵圆孔未闭、动脉导管未闭、房间隔缺损。结论:东莞市先心病发生率呈上升趋势,且高于全国平均水平;先心病发生率与孕母年龄、户籍、怀孕胎数、围产儿性别呈相关性;先心病的发生以单一类型为主,东莞市今后仍需进一步提高对先心病的监测水平及监测信息的准确性,降低先心病的漏报率,以获得更真实可靠的资料。
Objective: To understand the status of perinatal congenital heart disease (CHD) in Dongguan and to provide basis for further developing intervention measures for CHD. Methods: From January 2008 to December 2011, 556 282 perinatal infants who were hospitalized for delivery from 28 weeks pregnant to 7 days postpartum in Dongguan township were monitored for birth defects. The data of CHD were analyzed. Results: ① In 4 years, Dongguan City Hospital monitored 1 702 cases of perinatal congenital heart disease, the average incidence was 3.06 ‰. ② The incidence of CHD has the following characteristics: The incidence of CHD among pregnant women aged 30-35 years old is significantly higher than that of other age groups (P <0.01); twin (or multiple) pregnancy CHD occurs (P <0.01). The incidence of prenatal congenital heart disease in Dongguan was higher than that of non-Dongguan pregnant women, the difference was statistically significant (P <0.01). In male patients with congenital heart disease The incidence of congenital heart disease was higher than that of the female (P <0.01). ③The congenital heart disease complicated with other deformities was 127, accounting for 7.46% of the congenital heart disease. ④ The first three types of congenital heart disease were as follows: Closed, patent ductus arteriosus, atrial septal defect. Conclusion: The prevalence of congenital heart disease in Dongguan shows an upward trend, which is higher than the national average. The incidence of congenital heart disease is related to the age of pregnant women, household registration, number of pregnant fetuses and perinatal sex. The incidence of congenital heart disease is mainly of a single type , Dongguan City, the future still need to further improve the monitoring of congenital heart disease and monitoring information accuracy, reduce the rate of omission of congenital heart disease in order to obtain more realistic and reliable information.
