目的对涉及淋巴瘤与良性病变在扩散加权成像中的应用进行文献复习汇总,定量评价表观扩散系数(ADC)值的鉴别作用。方法检索Pubmed、中国生物医学文献数据库、中国学术期刊网全文数据库、万方数据库、Embase 2004年到2016年公开发表的中、英文文献,按照统一的纳入及排除标准筛选文献,提取出相关参数及ADC值的平均值和标准差,汇总标准化均数差(SMD)及其95%可信区间。结果共纳入27篇文献,其中英文12篇,中文15篇,所有纳入文献的淋巴瘤与良性病变ADC的SMD为-2.25(-2.62,-1.89),对发生部位进行亚组分析,发生于中枢神经系统的淋巴瘤与良性病变ADC的SMD为-1.98(-2.65,-1.32),发生于全身淋巴结者为-2.44(-2.81,-2.07);对场强大小进行亚组分析,1.5 T下淋巴瘤与良性病变ADC的SMD为-2.28(-2.69,-1.87),3.0 T下为-2.22(-3.03,-1.40)。所有SMD结果均有统计学意义。结论 ADC可以用于淋巴瘤与良性病变的鉴别,并且淋巴瘤的ADC值更低。
OBJECTIVE: To review and summarize the applications of lymphoma and benign lesions in diffusion-weighted imaging and to quantitatively evaluate the differential value of apparent diffusion coefficient (ADC). Methods PubMed, Chinese Biomedical Literature Database, Chinese Academic Journal Net Full-text Database, Wanfang Database, Embase published in Chinese and English literature from 2004 to 2016 were selected according to uniform inclusion and exclusion criteria, and relevant parameters were extracted Mean and standard deviation of ADC values, summary of standardized mean difference (SMD) and its 95% confidence interval. Results A total of 27 articles were included, including 12 articles in English and 15 in Chinese. The ADCs of all included articles were -2.25 (-2.62, -1.89) for ADC of lymphoma and benign lesions. The subgroups were analyzed in the central part The ADCs of neurological lymphomas and benign lesions were -1.98 (-2.65, -1.32) for ADC and -2.44 (-2.81, -2.07) for all-cell lymph nodes. Subgroup analyzes of field strengths were performed at 1.5 T The SMD of ADC for lymphoma and benign lesions was -2.28 (-2.69, -1.87) and -2.22 (-3.03, -1.40) at 3.0 T. All SMD results were statistically significant. Conclusion ADC can be used to differentiate lymphomas from benign lesions, and lymphoma has lower ADC value.
