目的探讨骶管麻醉在小儿下肢手术中应用的临床效果和安全性。方法选择60例行下肢手术的患儿随机分为骶管麻醉(C)组及氯胺酮麻醉(K)组,每组各30例。观察并记录MAP、HR、RR、SpO2,以及术后清醒时间,术后躁动、恶心呕吐不良反应的发生率。结果 C组MAP、HR、RR都比较平稳。K组手术开始后MAP、HR与麻醉前相比显著增高增快(P<0.05),与C组相比明显增高增快(P<0.05)。C组比K组术后清醒快(P<0.01),躁动呕吐不良反应少(P<0.05)。结论骶管麻醉用于小儿下肢手术安全,麻醉效果满意,呼吸循环稳定,不良反应少。
Objective To investigate the clinical effect and safety of caudal anesthesia in pediatric lower limb surgery. Methods Sixty children undergoing lower extremity surgery were randomly divided into caudal anesthesia group (C) and ketamine anesthesia (K group), 30 in each group. The incidences of MAP, HR, RR, SpO2, postoperative wakefulness, postoperative agitation and nausea and vomiting were observed and recorded. Results C, MAP, HR, RR were relatively stable. MAP and HR in group K were significantly higher than those before anesthesia (P <0.05), but were significantly higher than those in group C (P <0.05). Patients in group C were more awake than those in group K (P <0.01), and had few adverse reaction to agitation and vomiting (P <0.05). Conclusion caudal anesthesia for the safety of pediatric lower limb surgery, satisfactory anesthesia, respiratory cycle stability, fewer adverse reactions.
