目的探讨原发性高血压合并2型糖尿病患者血压变异性(BPV)和肱踝脉搏波传导速度(baPWV)的特点及科素亚对其影响。方法 60例原发性高血压合并2型糖尿病患者设为治疗组,口服氯沙坦钾片(100~200 mg·d-1)24周,观察治疗前后BPV、baPWV的变化,并与正常对照组(58例)比较。结果原发性高血压合并2型糖尿病患者和正常对照组比较BPV(24 h、日间、夜间、SBP和DBP的血压变异性)明显增大,baPWV明显增快。口服氯沙坦钾片(100~200 mg·d-1)24周治疗后24 h、日间、夜间、SBP和DBP的血压变异性与治疗前比较均有统计学差异,治疗后血压变异性明显减小(P<0.01),baPWV明显降低(P<0.01)。结论氯沙坦钾片能降低血压和改善原发性高血压合并2型糖尿病患者血压变异性,同时还有改善患者血管弹性的作用。
Objective To investigate the characteristics of blood pressure variability (BPV) and brachial-ankle pulse wave velocity (baPWV) in patients with essential hypertension and type 2 diabetes mellitus (T2DM) and the effect of Cozaar on it. Methods Sixty patients with essential hypertension and type 2 diabetes were treated with losartan potassium (100-200 mg · d-1) for 24 weeks. The changes of BPV and baPWV before and after treatment were observed and compared with those of normal control Group (58 cases) comparison. Results BPV (blood pressure variability of SBP and DBP at 24 h, daytime, and nighttime) were significantly increased and baPWV was significantly increased in patients with primary hypertension complicated with type 2 diabetes mellitus and normal controls. Blood pressure variability of oral losartan potassium tablets (100 ~ 200 mg · d-1) 24 weeks, daytime and nighttime SBP and DBP after 24 weeks of treatment were significantly different from those before treatment, after treatment, blood pressure variability (P <0.01), and baPWV decreased significantly (P <0.01). Conclusion Losartan potassium tablets can lower blood pressure and improve blood pressure variability in patients with essential hypertension complicated with type 2 diabetes mellitus, meanwhile, it can also improve blood vessel elasticity of patients.
