目的探讨产前超声诊断胎儿肾脏异常与妊娠结局的关系。方法对2004年3月~2013年7月产前超声筛查提示胎儿肾脏异常,并且在复旦大学附属妇产科医院产前诊断中心进行多科会诊的305例孕妇进行产前及新生儿随访观察。结果305例孕妇中有37例失访;2例妊娠中期流产;3例妊娠晚期胎死宫内;64例引产终止妊娠,其中30例为优生引产;199例分娩并随访至新生儿期,其中新生儿死亡4例、需手术治疗20例,余175例转归良好。结论产前B超提示胎儿肾脏异常不是优生引产的指征,排除致死性多发畸形和胎儿染色体疾病后,仅约14.2%的胎儿预后不良。
Objective To investigate the relationship between prenatal ultrasound diagnosis of fetal kidney abnormalities and pregnancy outcome. Methods From March 2004 to July 2013, prenatal ultrasound screening revealed fetal kidney abnormalities, and 305 pregnant women with multidisciplinary consultations at prenatal diagnosis center of Obstetrics and Gynecology Hospital of Fudan University were followed up for prenatal and neonatal follow-up . RESULTS: Of the 305 pregnant women, 37 were lost to follow-up; 2 were miscarried during the second trimester of pregnancy; 3 were in the third trimester of pregnancy; 64 were terminated by induction of labor; 30 were prenatal abortion; 199 were delivered and were followed up to the neonatal period, Neonatal death in 4 cases, requiring surgical treatment of 20 cases, more than 175 cases of good prognosis. Conclusions Prenatal B-mode ultrasound suggests that fetal abnormalities in the fetus are not indications for eutopic pregnancy. Only 14.2% of fetuses have poor prognosis after excluding fatal multiple malformations and fetal chromosomal diseases.
