我院自1992年3月～1995年1月,应用经内镜注射硬化剂对26例肝硬化食管静脉曲张及出血患者进行治疗,现总结如下.1 资料和方法1.1 病例选择 本组26例,肝炎后肝硬化25例,酒精性肝硬化1例.男23例,女3例.年龄20～50岁,平均40岁.上消化道出血22例,4例无出血史.本组病例均无肝性脑病及心肺功能衰竭.1.2 治疗方法 术前准备:采用日本PENT AX FG-34JA型胃镜及NM-1K注射针.硬化剂采用国产5%鱼肝油酸钠.急症出血时先放置三腔管压迫止血并补充血容量、纠正休克,同时给小剂量垂体后叶素静脉滴注.术前当日禁食禁水,术前15分钟肌注硫酸阿托品0.5mg,必要时肌注安定10mg,口服4%利多卡因5ml咽部粘膜麻醉.操作方法:插入纤维胃镜首先观察食管静脉曲张及出血的部位.对急性出血病人采用静脉内和静脉粘膜下注射法,即发现出血点后,先选择出血点的远侧端静脉两侧粘膜下注入,再选择出血部位近侧端静脉中央静脉内注射.对静止期硬化剂治疗时,采用静脉周围粘膜下点状注射法,开始第一针部 Our hospital from March 1992 to January 1995, the application of endoscopic injection of sclerotherapy on 26 patients with cirrhosis esophageal varices and bleeding patients are summarized as follows.1 Materials and Methods 1.1 Case Selection This group of 26 patients, Hepatitis cirrhosis in 25 cases, alcoholic cirrhosis in 1 case, 23 males and 3 females, aged 20 to 50 years old, average 40 years old, 22 cases of upper gastrointestinal bleeding, 4 cases without history of bleeding in this group of patients were Hepatic encephalopathy and cardiopulmonary failure .1.2 treatment preoperative preparation: Japan PENT AX FG-34JA gastroscope and NM-1K injection needle.
