目的:探讨微创拔牙结合牙龈成形术在义齿美观修复中的作用。方法:把40例需拔除牙随机分为治疗组20例和对照组20例,治疗组采用微创拔牙术,并在拔牙后安装以临牙为基牙的临时义齿。对照组采用常规拔牙术,术后不安装临时义齿。比较2组牙槽突形态及龈缘形态的改变情况。结果:治疗组牙槽突形态A级14例,B级5例,C级1例,无1例呈D级及E级。对照组牙槽突形态A级5例,B级8例,C级4例,D级2例,E级1例。治疗组19例龈缘形态呈扇贝形,对照组无一例龈缘呈扇贝形。结论:微创拔牙结合牙龈成形术可最大限度保持牙槽骨骨量及牙龈正常形态,取得义齿与牙龈间协调美观的修复效果。
Objective: To investigate the effect of minimally invasive extraction combined with gingivaloplasty in aesthetic restoration of dentures. Methods: Forty patients were randomly divided into treatment group (n = 20) and control group (n = 20). The treatment group was treated by minimally invasive extraction and the provisional denture with temporary incisors was installed after extraction. The control group using conventional extraction surgery, do not install temporary dentures. The changes of alveolar shape and gingival margin were compared between the two groups. Results: Alveolar process in the treatment group, 14 cases of grade A, B grade in 5 cases, C grade in 1 case, none of the cases were D and E grade. Alveolar process in control group, 5 cases of grade A, B grade in 8 cases, C grade in 4 cases, D grade in 2 cases, E grade in 1 case. In the treatment group, gingival margin was scallop in 19 cases, while in the control group, there was no scallop in gingival margin. Conclusion: Minimally invasive extraction combined with gingivaloplasty can keep the alveolar bone mass and the normal shape of the gums, and achieve a harmonious and esthetic restoration effect between the denture and the gingival.
