目的:探讨氧化型α1-抗胰蛋白酶(ox-AAT)、高迁移率族蛋白B1(HMGB1)、基质金属蛋白酶-9(MMP-9)在胎膜早破(PROM)孕妇中的表达及意义,为胎膜早破孕妇的早期诊断和治疗提供参考依据。方法:收集我院2016年1月到2017年1月收治的76例足月胎膜早破(TPROM)、44例未足月胎膜早破(PPROM)及50例正常足月孕妇作为研究对象,采用ELISA法检测母血及新生儿脐血ox-AAT、HMGB1、MMP-9水平,采用免疫组化法检测胎膜组织中ox-AAT、HMGB1、MMP-9蛋白的表达,比较三组母血、新生儿脐血及胎膜组织中ox-AAT、HMGB1、MMP-9水平,分析胎膜组织中ox-AAT、HMGB1、MMP-9之间相关性。结果:TPROM组与PPROM组母血、新生儿脐血、胎膜组织中ox-AAT、HMGB1、MMP-9水平均显著高于对照组(P<0.05),且PPROM组以上指标均显著高于TPROM组(P<0.05)。胎膜组织中MMP-9的表达与ox-AAT呈正相关(r=0.779,P<0.05),与HMGB1的表达呈正相关(r=0.811,P<0.05)。结论:母血、新生儿脐血、胎膜组织中ox-AAT、HMGB1、MMP-9变化与PROM及早产的发生相关,胎膜组织中MMP-9、ox-AAT、HMGB1均可能作为临床胎膜早破诊断以及治疗效果评价的参考指标。抗胰蛋白酶
Objective: To investigate the expression and significance of ox-AAT, HMGB1 and MMP-9 in pregnant women with premature rupture of membranes (PROM) , Provide a reference for early diagnosis and treatment of premature rupture of membranes in pregnant women. Methods: Totally 76 TPMS cases, 44 cases of incomplete premature rupture of membranes (PPROM) and 50 normal term pregnant women were enrolled in our hospital from January 2016 to January 2017 as study subjects The levels of ox-AAT, HMGB1 and MMP-9 in maternal blood and neonates were measured by ELISA. The expression of ox-AAT, HMGB1 and MMP-9 in fetal membranes was detected by immunohistochemical method. The levels of ox-AAT, HMGB1 and MMP-9 in umbilical cord blood and fetal membranes were detected by ELISA. The correlation between ox-AAT, HMGB1 and MMP-9 in fetal membranes was analyzed. Results: The levels of ox-AAT, HMGB1 and MMP-9 in maternal serum, neonatal cord blood and fetal membranes in TPROM group and PPROM group were significantly higher than those in control group (P <0.05), and those in PPROM group were significantly higher than those in PPROM group TPROM group (P <0.05). The expression of MMP-9 in fetal membranes was positively correlated with ox-AAT (r = 0.779, P <0.05), and positively correlated with the expression of HMGB1 (r = 0.811, P <0.05). CONCLUSION: The changes of ox-AAT, HMGB1 and MMP-9 in maternal blood, neonatal cord blood and fetal membranes are related to the occurrence of PROM and preterm birth. MMP-9, ox-AAT and HMGB1 in fetal membranes may be used as clinical Membrane premature rupture diagnosis and evaluation of the therapeutic effect of reference indicators. Antitrypsin
