目的探究在慢性阻塞性肺疾病(简称慢阻肺)急性加重患者中,肺动脉主干直径与主动脉直径比值(PA∶A)>1与血浆尿钠肽(BNP)、C反应蛋白(CRP)、白细胞介素6(IL-6)水平的相关性。方法纳入2013年8月至2013年12月于华西医院呼吸与危重症医学科住院治疗的慢阻肺急性加重患者95例,根据PA∶A是否>1分为两组:PA∶A>1组和PA∶A≤1组。收集两组患者的基本资料,并检测入院24 h内动脉血气分析、血浆BNP、CRP和IL-6水平。结果 PA∶A>1组患者血浆BNP水平为483~4 582 ng/L,中位数为2 005 ng/L;PA∶A≤1组患者血浆BNP水平为137~1 224 ng/L,中位数为404 ng/L,两组患者的血浆BNP水平比较有显著差异(P<0.01),CRP、IL-6水平差异均无统计学意义。结论在慢阻肺急性加重患者中,PA∶A>1与BNP水平有相关性,但与CRP、IL-6无相关性。
Objective To investigate the relationship between the ratio of pulmonary artery diameter to aortic diameter (PA:A)> 1 and plasma natriuretic peptide (BNP) and C-reactive protein (CRP) in patients with acute exacerbation of chronic obstructive pulmonary disease Interleukin-6 (IL-6) levels were examined. Methods Ninety-five patients with chronic obstructive pulmonary disease who were hospitalized in Department of Respiratory and Critical Care Medicine, West China Hospital from August 2013 to December 2013 were divided into two groups according to whether PA:A> 1: PA: A> 1 And PA: A≤1 group. The basic data of two groups of patients were collected, and arterial blood gas analysis, plasma BNP, CRP and IL-6 levels within 24 hours after admission were detected. Results The plasma BNP levels of patients with PA:A> 1 were 483-4 582 ng / L, with a median of 2 005 ng / L. Plasma BNP levels were 137-1 224 ng / L in patients with PA: A ≤ 1, The median of plasma BNP level was 404 ng / L in both groups (P <0.01). There was no significant difference in CRP and IL-6 levels between the two groups. Conclusion In patients with acute exacerbation of chronic obstructive pulmonary disease, PA: A> 1 has a correlation with BNP level, but not with CRP, IL-6.
