目的探讨代谢综合征(metabolic syndrome,MS)及其各组分与无症状脑梗死(silent cerebral infarction,SCI)的相关性。方法连续收集我科2006年5月至2010年6月健康体检患者959例[男性693例,女性266例,年龄(49.8±9.1)岁],根据修订后的中国人代谢综合征诊断标准,将所有患者分为代谢综合征组(443例)和非代谢综合征组(516例)。比较两组患者无症状脑梗死的患病率并采用单因素和多因素Logistic回归法分析代谢综合征及其各组分与无症状脑梗死的相关性。结果单因素统计分析发现代谢综合征组罹患无症状脑梗死的比例明显高于非代谢综合征组(41.1%vs27.5%,P<0.000 1)。多因素Logistic回归分析显示代谢综合征与无症状脑梗死发病明显相关(OR=1.73,95%CI:1.24~2.41,P=0.001 2),在代谢综合征各组分内高血压(OR=1.40,95%CI:1.05~1.88,P=0.023 0)、糖尿病(OR=1.75,95%CI:1.23~2.48,P=0.001 9)也是无症状脑梗死的独立危险因素,而其他组分无统计学意义。结论代谢综合征与无症状脑梗死关系密切,代谢综合征是无症状脑梗死的独立危险因素。
Objective To investigate the relationship between metabolic syndrome (MS) and its components and silent cerebral infarction (SCI). Methods A total of 959 healthy volunteers (693 males and 266 females, aged 49.8 ± 9.1 years) from May 2006 to June 2010 in our department were continuously collected. According to the revised Chinese metabolic syndrome diagnostic criteria, All patients were divided into metabolic syndrome group (443 cases) and non-metabolic syndrome group (516 cases). The prevalence of asymptomatic cerebral infarction in both groups was compared and the correlation between metabolic syndrome and its components and asymptomatic cerebral infarction was analyzed by single factor and multivariate Logistic regression. Results The univariate statistical analysis showed that the proportion of asymptomatic cerebral infarction in the metabolic syndrome group was significantly higher than that in the non-metabolic syndrome group (41.1% vs27.5%, P <0.0001). Multivariate logistic regression analysis showed that the metabolic syndrome was significantly associated with the onset of asymptomatic cerebral infarction (OR = 1.73, 95% CI: 1.24-2.41, P = 0.001 2) , 95% CI: 1.05-1.88, P = 0.023 0). Diabetes mellitus (OR = 1.75, 95% CI: 1.23-2.48, P = 0.001 9) was also an independent risk factor for asymptomatic cerebral infarction Significance of learning. Conclusion Metabolic syndrome is closely related to asymptomatic cerebral infarction. Metabolic syndrome is an independent risk factor for asymptomatic cerebral infarction.
