目的:探讨可溶性细胞间黏附分子-1(sICAM-1)在鼻息肉发病中的作用及糖皮质激素治疗鼻息肉的机制。方法:采用双抗体夹心酶联免疫吸附法(ELISA)测定鼻息肉患者经糖皮质激素全身和局部治疗前、后血清中sICAM-1的含量变化,同时观察糖皮质激素对鼻息肉的治疗效果。结果:①鼻息肉患者血清中sICAM-1的含量高于正常对照组;②经糖皮质激素治疗后,鼻息肉患者血清中sICAM-1含量显著下降,显效组sICAM-1下降的程度大于有效组;③糖皮质激素治疗前、后,患者鼻息肉大小计分减少程度与血清中sICAM-1下降程度成正相关。结论:细胞间黏附分子-1(ICAM-1)在鼻息肉发生发展过程中起着重要作用。糖皮质激素可以通过对抗炎性细胞间黏附而对鼻息肉起治疗作用,血清中sICAM-1的水平变化可作为鼻息肉药物治疗疗效的临床判断指标。
Objective: To investigate the role of soluble intercellular adhesion molecule-1 (sICAM-1) in the pathogenesis of nasal polyps and the mechanism of glucocorticoid treatment of nasal polyps. Methods: Serum levels of sICAM-1 in patients with nasal polyps were measured by enzyme-linked immunosorbent assay (ELISA) before and after glucocorticoid therapy. Meanwhile, the therapeutic effects of glucocorticoid on nasal polyps were observed. Results: ①The content of sICAM-1 in the serum of patients with nasal polyps was higher than that of the normal control group. ② After the glucocorticoid treatment, the serum sICAM-1 level of patients with nasal polyps was significantly decreased, and the extent of sICAM-1 decreased significantly ; ③ glucocorticoid before and after treatment, the reduction of nasal polyps size score and serum levels of sICAM-1 is positively correlated. Conclusion: Intercellular adhesion molecule-1 (ICAM-1) plays an important role in the development of nasal polyps. Glucocorticoids can play a therapeutic role in the treatment of nasal polyps by combating inflammatory cell adhesion. The change of sICAM-1 level in serum can be used as a clinical judgment indicator for the therapeutic effect of nasal polyps.
