目的探讨急性脑梗死患者血浆脂蛋白相关磷脂酶A2(LP-PLA2)与神经功能缺损严重程度之间的关系。方法选择90例病程在72 h之内的急性脑梗死患者,用NIHSS量表于入院当天进行神经功能缺损程度评分,依据评分将患者为轻型、中型、重型3组。40例健康体检者作为对照组,两组均于入组时测定空腹血液LP-PLA2浓度。结果与对照组相比,急性脑梗死患者血液LP-PLA2浓度明显升高,差异有统计学意义(P<0.05)。轻型、中型、重型3组脑梗死患者间血液LP-PLA2浓度差异有统计学意义(P<0.01)。急性脑梗死组神经功能缺损程度的评分与血液LP-PLA2浓度呈正相关(r=0.482,P<0.05)。结论急性脑梗死患者血液LP-PLA2浓度明显升高,病情越严重,LP-PLA2水平越高,LP-PLA2可能是急性脑梗死患者病情严重程度风险预测的一个重要生物学标志物。
Objective To investigate the relationship between plasma lipoprotein associated phospholipase A2 (LP-PLA2) and the severity of neurological deficits in patients with acute cerebral infarction. Methods Ninety patients with acute cerebral infarction within 72 hours were scored on the NIHSS scale on the day of admission and were divided into three groups: light, medium and heavy. Forty healthy subjects were included as control group. LP-PLA2 concentration in fasting blood was measured in both groups before admission. Results Compared with the control group, the blood LP-PLA2 concentration in patients with acute cerebral infarction was significantly increased, the difference was statistically significant (P <0.05). The levels of LP-PLA2 in blood of patients with mild, moderate and severe cerebral infarction were significantly different (P <0.01). The score of neurological deficit in acute cerebral infarction group was positively correlated with LP-PLA2 concentration (r = 0.482, P <0.05). Conclusion The LP-PLA2 concentration in blood is significantly increased in patients with acute cerebral infarction. The more severe the disease, the higher the level of LP-PLA2. LP-PLA2 may be an important biomarker for predicting the severity of the disease in patients with acute cerebral infarction.
