目的观察甲状腺机能减退(甲减)患者血清脂质的变化及其意义,探讨甲状腺激素浓度改变对脂质水平的影响。方法对34例甲减患者和36例健康对照者,采用BECKMAN COULTER Access全自动微粒子化学发光免疫分析仪检测三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)。用OLYMPUS AU-2700全自动生化分析仪检测总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)。结果甲减患者组血清TC、LDL-C水平分别为5.80±1.62mmol/L、3.45±1.05mmol/L,对照组TC、LDL-C水平为5.05±1.00mmol/L、2.42±0.82mmol/L,两组比较,差异均有统计学意义(t=2.36,P<0.05;t=4.62,P<0.01),血清TG及HDL-C水平两组间差异无统计学意义(P>0.05)。相关分析,T3、FT3、FT4与TC及LDL-C水平呈明显负相关,TSH与TC、TG、LDL-C水平呈明显正相关。结论甲减患者TC及LDL-C水平明显升高,且与甲状腺激素水平密切相关,检测血清脂质可作为判断甲减患者病情及疗效的有效指标。
Objective To observe the changes of serum lipids in patients with hypothyroidism (hypothyroidism) and its significance, and to explore the effect of thyroid hormone concentration on lipid levels. Methods Thirty-four patients with hypothyroidism and 36 healthy controls were enrolled in this study. BECKMAN COULTER Access automated chemiluminescence immunoassay was used to detect triiodothyronine (T3), thyroxine (T4), free triiodothyronine (FT3), free thyroxine (FT4), thyroid stimulating hormone (TSH). Total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) were detected by OLYMPUS AU-2700 automatic biochemistry analyzer. Results The levels of TC and LDL-C in hypothyroidism patients were 5.80 ± 1.62mmol / L and 3.45 ± 1.05mmol / L, respectively. The levels of TC and LDL-C in control group were 5.05 ± 1.00mmol / L and 2.42 ± 0.82mmol / L There was significant difference between the two groups (t = 2.36, P <0.05; t = 4.62, P <0.01). There was no significant difference between the two groups in serum TG and HDL-C levels (P> 0.05). Correlation analysis showed that there was a significant negative correlation between T3, FT3, FT4 and TC and LDL-C levels, while TSH was positively correlated with TC, TG and LDL-C levels. Conclusions TC and LDL-C levels in patients with hypothyroidism are significantly increased, and are closely related to thyroid hormone levels. Detecting serum lipids can be used as an effective indicator to judge the severity and efficacy of hypothyroidism.
