目的探讨血清超敏C-反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、脂联素(APN)及血脂等变化与2型糖尿病下肢动脉病变的关系。方法对62例内分泌科诊治的老年2型糖尿病(T2DM)患者和30例在本院健康体检者进行研究。依据T2DM患者踝肱动脉指数(ABI)将糖尿病患者分为下肢动脉病变组(PAD组)和非PAD组。测定各组血清hs-CRP、IL-6、APN、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、血脂等水平的变化,并进行比较。结果 2型糖尿病下肢动脉病变患者hs-CRP、IL-6、FPG、HbA1c、血脂的水平显著高于对照组(P<0.05或P<0.01),APN显著低于对照组(P<0.01);PAD组hs-CRP、IL-6水平高于非PAD组(P<0.05或P<0.01),APN低于非PAD组(P<0.01)。结论 hs-CRP、IL-6、APN、血脂水平对于2型糖尿病下肢动脉病变的早期诊断和病情分析具有一定的临床价值。
Objective To investigate the relationship between serum hs-CRP, interleukin-6 (IL-6), adiponectin (APN) and serum lipids and lower extremity arterial disease in type 2 diabetes mellitus. Methods Sixty-two elderly patients with type 2 diabetes mellitus (T2DM) diagnosed by endocrinology and 30 healthy people undergoing physical examination in our hospital were studied. According to the ankle-brachial artery index (ABI) of T2DM patients, diabetic patients were divided into lower extremity arterial lesions group (PAD group) and non-PAD group. The changes of serum hs-CRP, IL-6, APN, fasting blood glucose (FPG), HbA1c and blood lipids were measured and compared. Results The levels of hs - CRP, IL - 6, FPG, HbA1c, and lipids in patients with type 2 diabetic lower extremity arterial disease were significantly higher than those of the control group (P <0.05 or P <0.01), and APN was significantly lower than that of the control group (P <0.01). The levels of hs-CRP and IL-6 in PAD group were higher than those in non-PAD group (P <0.05 or P <0.01), and those in APAD group were lower than those in non-PAD group (P <0.01). Conclusion The hs-CRP, IL-6, APN, blood lipid level have some clinical value for the early diagnosis and condition analysis of lower extremity arterial disease in type 2 diabetic patients.
