1990年以来,我们收治牙裂患者35例(年龄13~72岁),其中显性牙裂14例,隐性21例。第一磨牙裂26例,第一、二前磨牙裂6例,前门牙裂3例(均有外历史)。主要表现为咬(牙合)不适或咬(牙合)痛及冷热刺激痛、侧击痛等。显性牙裂以咬(牙合)痛常见,多表现为刺痛,查体可见牙冠裂纹或劈裂的牙片。隐性牙裂主要表现为咬(牙合)不适或冷热刺激痛等,一船检查很难发现裂纹,易误诊为其它牙病。 治疗方法及结果:21例隐性牙裂中,12例直接行裂纹充填(其中盖髓3例),6例行牙髓治疗,3例行根管治疗,均给予金属全冠修复。所有患牙均调低(牙合)关系。1年后夏查,除1例套冠脱落牙齿松动Ⅲ°而拔除外,其余牙冠均无症状而得以保留。14例显性牙裂中,5例因牙冠劈裂至龈下而予以拔除,另9例均行根管治疗,患牙套以金属全冠,调低(牙合)关系。1年后复查,4例无症状而患牙得以保留,3例因原有症状加重而自行拔除,2例复查时仍有咬(牙合)痛。
Since 1990, we treated 35 cases of patients with dental fractures (aged 13 to 72 years), including 14 cases of dominant dental fractures and 21 cases of recessiveness. The first molar fractures in 26 cases, the first and second premolar fractures in 6 cases, anterior incisors cracked in 3 cases (both outside history). Tactile fractures mainly manifested as bite (disability) or hot and cold pain, etc., a ship inspection is difficult to find cracks, easily misdiagnosed as other dental disease. Treatment methods and results: In 21 cases of recessive fissures, 12 cases were directly filled with cracks (3 cases of pulp capping), 6 cases were treated with endodontics and 3 cases were treated with root canal. All cases were given metal crown restoration. All teeth are lower (occlusion) relationship. After 1 year summer investigation, in addition to a case of crown off tooth loose Ⅲ ° and unplug, the rest of the crown are asymptomatic and retained. Of the 14 cases of dominant dental dissection, 5 cases were removed due to the splitting of the crown into the subgingival area. The other 9 cases underwent root canal treatment. The braces were crowned with metal and the occlusion was reduced. One year after the review, 4 cases of asymptomatic teeth to be retained, 3 cases due to the exacerbation of the original symptoms and their own removal, 2 cases were still bite (occlusion) pain.
