目的:比较羊水泡沫试验和650 nm时的光密度(OD650)测定评估胎肺成熟度(FLM)的诊断效能,旨在为产科医生选择恰当的FLM评估方法提供依据。方法:132例患新生儿呼吸窘综合征(NRDS)单胎高危孕妇在产时直视下羊膜穿刺术抽取羊水,采用χ2检验和Fisher精确检验法比较泡沫试验和OD650测定这两种方法的产前预测NRDS的准确度。结果:132例孕妇中分娩患NRDS的新生儿10例,无论泡沫试验还是OD650测定,在足月孕妇中的准确度明显高于早产孕妇,有统计学意义(P<0.05);无论是在早产还是足月孕妇中,OD650测定的准确度明显高于泡沫试验,差异有统计学意义(P<0.05)。结论:OD650测定在产前预测NRDS的准确度明显优于泡沫试验,且较泡沫试验更客观。我们推荐临床上32W以后FLM评估优先采用OD650测定,且孕龄越大OD650测定的可靠性越高。
OBJECTIVE: To assess the diagnostic efficacy of fetal lung maturity (FLM) in amniotic fluid foam test and optical density at 650 nm (OD650), aiming to provide evidence for obstetricians to choose the appropriate FLM assessment method. Methods: 132 amniotic fluid samples were obtained from amniocentesis by amniocentesis in 132 neonates with high birth weight infants with respiratory distress syndrome (NRDS) under amniocentesis. Compared with foam test and OD650 assay by χ2 test and Fisher exact test, Predict NRDS accuracy before. Results: Among 132 pregnant women, 10 cases of newborns with NRDS had no significant difference (P <0.05). The accuracy of foam test or OD650 test in full-term pregnant women was significantly higher than that of premature pregnant women (P <0.05) Or full-term pregnant women, OD650 determination accuracy was significantly higher than the foam test, the difference was statistically significant (P <0.05). CONCLUSION: The accuracy of OD650 in predicting NRDS during prenatal testing is significantly better than that of foam testing and more objective than that of foam testing. We recommend the clinical use of 32M FLM FLD priority given to the determination of OD650, and the greater the gestational age, the higher the reliability of the OD650 determination.
