目的观察前列环素 Ⅰ_2(PGI_2)雾化吸入治疗犬油酸型急性呼吸窘迫综合征(ARDS)对血液动力学、血气、呼吸功能及肺组织病理等影响。方法以0.11ml/kg油酸致犬急性肺损伤后,随机分为2组, Ⅰ组为对照组(n=5),单纯行机械通气治疗; Ⅱ组为PGI_2雾化吸入治疗组(n=6),分别以5、10、20、30ng·kg~-1·min~-1的PGI_2雾化吸入治疗15分钟,间隔15分钟;然后以20ng·kg~-1·min~-1持续雾化吸入 1小时。观察血液动力学指标、动脉血气、肺动态顺应性(Cdyn)等变化,同时进行肺组织病理学检查和肺组织湿/干(W/D)比值测定,综合评价治疗效果。结果 PGI_2 25ng· kg-1·min~-1即能显著增加 Cdyn,10~30ng· kg~-1· min~-1时,PAP显著降低(P<0. 05),MAP基本保持正常;20~30ng· kg~-1· min-1时,PaO_2显著升高,Qs/Qt则显著下降(P<0. 05);治疗后2小时,对照组PCWP升高,MAP降低(P<0.05),而实验组无显著变化。此外,实验组W/D显著下降(P<0.05);病理学检查肺组织损害明显减轻。结论PGI_2能选择?
Objective To observe the effects of inhalation of prostacyclin Ⅰ_2 (PGI_2) on hemodynamics, blood gas, respiratory function and lung histopathology in dogs with oleic acid acute respiratory distress syndrome (ARDS). Methods Acute lung injury induced by 0.11ml / kg oleic acid was divided into two groups at random: group Ⅰ (n = 5) and mechanical ventilation alone; group Ⅱ (n = 5) = 6), inhaled for 15 minutes with PGI_2 of 5, 10, 20, 30 ng · kg -1 · min -1, respectively, for 15 minutes and then continued for 20 ng · kg -1 · min -1 Inhalation for 1 hour. The changes of hemodynamics, arterial blood gas and dynamic compliance of lung (Cdyn) were observed. The lung histopathology and W / D ratio of lung tissue were also measured to evaluate the therapeutic effect. Results PGI 2 at 25 ng · kg -1 · min -1 could significantly increase Cdyn, MAP decreased at 10 ~ 30 ng · kg -1 -1 min -1 (P <0.05) PaO2 was significantly increased and Qs / Qt was significantly decreased (P <0.05) at 30ng kg-1min-1. PCWP was increased and MAP was decreased at 2h after treatment in control group (P <0. 05), while no significant changes in the experimental group. In addition, the experimental group W / D decreased significantly (P <0.05); pathological examination significantly reduced lung tissue damage. Conclusion PGI_2 can choose?
