目的研究原发性高血压患者运动血压与踝臂动脉脉搏波传导速度(baPWV)的关系。方法入选2008-05-2009-03在我院就诊的原发性高血压患者125例,予以行活动平板与baPWV的检查,根据运动血压结果分为运动高血压组(运动收缩压峰值≥200mmHg,n=36)和运动血压正常组(运动收缩压峰值<200mmHg,n=89),应用自动脉搏波传导速度测定仪测量baPWV作为反映大动脉硬度的指标。结果运动高血压组患者baPWV显著大于运动血压正常组[(14.9±1.3)比(13.0±0.9)m/s,P<0.01]。亚极量运动收缩压与baPWV密切相关(r=0.645,P<0.01),Logistic回归分析显示,baPWV(OR1.022,95%CI1.012~1.032)、静息收缩压[1.127(1.013~1.254)]、体质量指数[1.368(1.049~1.783)],均为影响运动高血压的独立危险因素。结论原发性高血压人群中运动血压与baPWV密切相关,大动脉硬化可能是导致运动高血压的重要危险因素之一。
Objective To study the relationship between ambulatory blood pressure and ankle brachial artery pulse wave velocity (baPWV) in patients with essential hypertension. Methods Selected from 2008-05-2009-03 in our hospital for treatment of 125 cases of patients with essential hypertension, to conduct activities of the plate and baPWV examination, according to exercise blood pressure results were divided into exercise-induced hypertension group (peak systolic blood pressure ≥ 200mmHg, n = 36) and exercise normotensive group (peak systolic blood pressure <200 mmHg, n = 89). The baPWV was measured by automatic pulse wave velocity meter as an indicator of aortic stiffness. Results The baPWV in exercise-induced hypertension group was significantly higher than that in exercise-induced hypertension group [(14.9 ± 1.3) vs (13.0 ± 0.9) m / s, P <0.01]. Suboptimal exercise systolic pressure was closely related to baPWV (r = 0.645, P <0.01). Logistic regression analysis showed that baPWV (OR1.022, 95% CI1.012-1.032), resting systolic pressure [1.127 )], Body mass index [1.368 (1.049 ~ 1.783)] were independent risk factors for exercise-induced hypertension. Conclusion The exercise-induced hypertension is closely related to baPWV in patients with essential hypertension. Atherosclerosis may be one of the important risk factors for exercise-induced hypertension.
