目的探讨整合素α_5β_1(integrinα_5β_1)、纤维连接蛋白(Fn)及CD_(44v6)与卵巢癌淋巴结转移的关系。方法用免疫组织化学Elivision法检测80例卵巢上皮性肿瘤中integrinα_5β_1、Fn及CD_(44v6)的表达;运用图像分析系统对各指标进行定量检测。结果定性及半定量结果:integrinα_5β_1、Fn的表达按良性、交界性及恶性的顺序呈递减趋势,组间差异均有统计学意义(P<0.05);CD_(44v6)的表达则呈递增趋势,组间差异有统计学意义(P<0.05);integrinα_5β_1、Fn淋巴结转移组与未转移组间表达差异有统计学意义(P<0.05);integrinα_5β_1与Fn的表达呈正相关,与CD_(44v6)的表达呈负相关。定量检测结果:inte- grinα_5β_1表达的阳性细胞面积按良性、交界性、恶性的顺序呈递减趋势,灰度呈递增趋势;CD_(44v6)的表达则相反,面积呈递增趋势,灰度呈递减趋势。结论integrinα_5β_1、Fn表达下降或缺失,CD_(44v6)表达升高,提示肿瘤进展、恶性度高、预后不良;计算机图像分析系统对卵巢上皮性肿瘤的自动化诊断、淋巴结转移及预后评估有重要意义。
Objective To investigate the relationship between integrinα_5β_1, fibronectin (Fn) and CD_ (44v6) and lymph node metastasis in ovarian cancer. Methods The expressions of integrinα_5β_1, Fn and CD_ (44v6) in 80 cases of ovarian epithelial neoplasms were detected by immunohistochemical Elivision method. The quantitative analysis of each index was carried out by image analysis system. Results The results of qualitative and semi-quantitative analysis showed that the expression of integrinα_5β_1 and Fn decreased gradually in the order of benign, borderline and malignant (P <0.05), while the expression of CD_ (44v6) showed an increasing trend, (P <0.05). There was significant difference between integrinα5β1, Fn lymph node metastasis group and non-metastasis group (P <0.05). There was a positive correlation between integrinα5β1 and Fn expression, Expression was negatively correlated. Quantitative detection results showed that the area of positive cells expressed by integrain-5β_1 showed a decreasing trend in the order of benign, borderline and malignant, and the grayscale showed an increasing trend; while the expression of CD_ (44v6) was opposite, the area showed an increasing trend and the grayscale showed a decreasing trend . Conclusion The expression of integrinα_5β_1, Fn is decreased or absent, and the expression of CD_ (44v6) is increased, suggesting that the tumor progression, malignancy and prognosis are poor. The computerized image analysis system is of great significance for the automated diagnosis of ovarian epithelial neoplasms, lymph node metastasis and prognosis.
