目的:分析平板运动试验(TET)与冠状动脉造影(CAG)的结果,探讨TET对冠心病的诊断价值。方法:选择396例有胸闷、胸痛的患者,进行TET和CAG,对比其结果,分析TET的敏感性和特异性。结果:396例患者中,TET阳性192例,其中CAG阳性158例,CAG阴性34例。TET阴性204例,其中CAG阴性156例,阳性48例。即TET的敏感性为76.7%,特异性为82.1%,预测准确性为81.35%,假阳性率为17.9%,假阴性率为23.3%。双支、3支冠状动脉病变的TET敏感性为81%和95%均明显高于单支病变的63.5%。女性TET假阳性率高于男性(P<0.05)。结论:TET诊断冠心病的敏感度和特异性较高,并可估测冠状动脉病变程度,且操作简单,是目前诊断冠心病较理想的非创性检查方法,适合临床广泛应用。
Objective: To analyze the results of treadmill exercise test (TET) and coronary angiography (CAG) and explore the diagnostic value of TET in coronary heart disease. Methods: A total of 396 patients with chest tightness and chest pain were selected for TET and CAG. The results were compared and the sensitivity and specificity of TET were analyzed. Results: Of the 396 patients, 192 were positive for TET, of which 158 were positive for CAG and 34 were negative for CAG. TET negative 204 cases, of which 156 cases of negative CAG, positive in 48 cases. That is, TET has a sensitivity of 76.7%, a specificity of 82.1%, a prediction accuracy of 81.35%, a false positive rate of 17.9% and a false negative rate of 23.3%. The double-vessel and triple-vessel coronary artery lesions had a TET sensitivity of 81% and 95%, respectively, which was significantly higher than 63.5% of single vessel lesions. The false positive rate of female TET was higher than that of male (P <0.05). Conclusion: TET is a sensitive and specific method for the diagnosis of coronary heart disease. It can estimate the severity of coronary artery disease and is easy to operate. It is an ideal noninvasive method for the diagnosis of coronary heart disease and is suitable for clinical application.
