目的:探讨介入放射学方法留置导管药盒治疗肝肿瘤的方法和效果。材料与方法:采用Seldinger技术经左锁骨下动脉穿刺将导管插入肝动脉,然后与植入式药盒连接并埋置于皮下,对31例原发性和转移性肝肿瘤患者经导管药盒系统进行了多次动脉灌注和栓塞化疗,灌注抗癌药物有ADM和CDDP,栓塞采用MMC-碘油乳剂。结果:有30例病人导管超选择置入肝左右动脉、肝固有动脉或肝总动脉,1例导管留置在腹腔动脉。患者术后平均生存期11.4月。本组无严重并发症发生。结论:植入式导管药盒系统非常实用,为晚期肝肿瘤患者的动脉灌注和栓塞化疗提供了一个永久和方便的治疗途径。
Objective: To explore the methods and effects of interventional radiology indwelling catheter kits for treating liver tumors. Materials and Methods: The Seldinger technique was used to insert the catheter into the hepatic artery via the left subclavian artery puncture, and then it was connected to the implanted cartridge and buried subcutaneously. The transcatheter kit system was used in 31 patients with primary and metastatic liver tumors. Several arterial infusions and embolization chemotherapy were performed. The infusion of anticancer drugs was ADM and CDDP, and embolization was performed with MMC-lipiodol emulsion. RESULTS: Thirty patients had superselective catheterization of the left and right hepatic arteries, hepatic proper arteries, or common hepatic artery. One catheter was placed in the celiac artery. The average survival time of patients was 11.4 months. No serious complications occurred in this group. Conclusion: The implantable catheterization kit system is very practical and provides a permanent and convenient treatment for arterial infusion and embolization chemotherapy in patients with advanced liver cancer.
