对氨试剂测定腺苷脱氨酶的方法进行了改进 ,并测定了 95例病人脑脊液腺苷脱氨酶 (ADA)活性 ,以探讨其对结核性脑膜炎的临床意义。病人分为 4组 :结核脑膜炎 2 5例 ;病毒性脑膜炎 43例 ;化脓性脑膜炎 9例 ;其他疾病 18例。结果分别为 :(14.6 8± 9.5 3)u/L、(3.38± 0 .96 )u/L、(5 .2 3± 3.2 3)u/L及 (2 .99± 1.32 )u/L。结脑组与非结脑组相比差异非常显著 (P <0 .0 1)。若以 6u/L为阳性界限值 ,脑脊液ADA检测对结脑病人的特异性为 98.9% ,敏感性 96 .0 %。脑脊液ADA活性测定对于结脑与非结脑的诊断和鉴别诊断具有重要价值。改进后的脑脊液ADA测定方法更为简便、快速、灵敏 ,适合于临床常规检测。
The method of measuring adenosine deaminase with ammonia reagent was improved and the activity of adenosine deaminase (ADA) in cerebrospinal fluid of 95 patients was measured to explore its clinical significance to tuberculous meningitis. Patients were divided into 4 groups: 25 cases of tuberculous meningitis; viral meningitis in 43 cases; purulent meningitis in 9 cases; other diseases in 18 cases. The results were (14.68 ± 9.53) u / L, (3.38 ± 0.96) u / L, (5.23 ± 3.23) u / L and (2.99 ± 1.32) u / L, respectively. The difference between tuberculin group and non-tuberculous group was significant (P <0.01). If 6u / L as a positive limit, cerebrospinal fluid ADA detection of tuberculous meningitis in patients with specificity of 98.9%, a sensitivity of 96.0%. Measurement of cerebrospinal fluid ADA activity for the diagnosis of tuberculous and non-tuberculous diagnosis and differential diagnosis of great value. The improved method of ADS determination of cerebrospinal fluid is more simple, rapid and sensitive, suitable for clinical routine testing.
