目的:探讨白内障超声乳化术中后囊破裂时残留晶体核的处理方法。方法:回顾性地研究18例(18眼)超声乳化术中后囊穿破时残留晶体核的处理结果。介绍低流量、低灌注、连续超声乳化法;注入高弹性粘弹性剂法;无灌注前段玻璃体切除(“干切”)等三种防止晶体核下沉的方法。随访12~24个月,平均18.2个月。结果:除1例特殊患者外,均未发生核下沉,且均一期植入后房型人工晶体。随访时视力达到0.5者有15眼(83.3%)。手术并发症主要是角膜水肿、黄斑囊样水肿等。结论:3种处理方法适应于不同类型的残留核的处理,且安全,可靠,并发症少,值得临床推广。眼科学报2000;16:106-108。
Objective: To investigate the treatment method of residual crystal nucleus in posterior capsule rupture during cataract phacoemulsification. Methods: Retrospective study of 18 cases (18 eyes) phacoemulsification of posterior capsule puncture residual nucleus processing results. Introduction of low-flow, low perfusion, continuous phacoemulsification; injection of high elastic viscoelastic agent method; without perfusion of the anterior vitrectomy (“dry cut”) and other three methods to prevent the crystal nuclei sink. Follow-up 12 to 24 months, an average of 18.2 months. Results: Except for one case of special patient, no nuclear subsidence occurred, and all posterior chamber intraocular lens implantation was performed. There were 15 eyes (83.3%) with visual acuity of 0.5 at follow-up. Surgical complications mainly corneal edema, cystoid macular edema and so on. Conclusion: The three treatment methods are suitable for the treatment of different types of residual nucleus, and they are safe, reliable and have fewer complications.
