目的 研究粘附分子E-Cadherin(E-CD)在乳腺癌中的表达及其与临床病理特征及术野脱落癌细胞友生率的关系,以求更加深入地探讨乳腺癌侵袭和转移的机制,为乳腺癌临床的诊治提供参考.方法 在以往乳腺癌手术野脱落癌细胞临床研究及对临床病例的配对分组的基础上,采用免疫组织化学ABC法,研究E-CD在52例乳腺浸润性导管癌组织中的表达强度.结果 在52例乳腺癌中E-CD正常表达者15例(28.9%),减弱表达者14例(26.9%),表达缺失23例(44.2%),总减弱或缺失的异常表达率为71.1%.E-CD的表达与组织学分级显著相关,I级的减弱或缺失表达率(36.4%)明显低于Ⅱ级(75%)和Ⅲ级(92.3%)(P<0.05),脱落细胞阳性组的E-CD异常表达率(84.6%)显著高于脱落细胞阴性组(57.7%)(P<0.05),E-CD的表达未发现与年龄大小、肿瘤直径、病理分期、腋淋巴结状态有显著相关.结论 乳腺癌普遍存在E-CD的表达缺陷;E-CD的表达与乳腺肿瘤的组织学分级密切相关,分化越差,肿瘤细胞E-CD的下调或缺失表达率越高;粘附分子E-CD所代表的肿瘤细胞间的粘附性与乳腺癌术野脱落癌细胞间有关系,E-CD缺失或下调表达率高者,越易脱落至手术野.
Objective To investigate the expression of adhesion molecule E-Cadherin (E-CD) in breast cancer and its relationship with clinicopathological features and the haplotype of detached cancer cells in the operative field in order to explore the mechanism of breast cancer invasion and metastasis in more depth. To provide reference for clinical diagnosis and treatment of breast cancer. METHODS: Based on previous clinical studies of breast cancer surgical field shedding cancer cells and pairing of clinical cases, immunohistochemical ABC method was used to study E-CD in 52 cases of breast invasiveness. Expression intensity in ductal carcinoma tissues. Results Among the 52 cases of breast cancer, 15 cases (28.9%) were normal E-CD expression, 14 cases (26.9%) were weakened, and 23 cases (44.2%) were missing. The abnormal expression rate of deletion was 71.1%. The expression of E-CD was significantly correlated with histological grade. The grade I attenuation or deletion rate (36.4%) was significantly lower than that of grade II (75%) and grade III (92.3%) ( P<0.05). The abnormal expression rate of E-CD (84.6%) in the positive group was significantly higher than that in the negative group (57.7%) (P<0.05). The expression of E-CD was not associated with age and tumor diameter. There was a significant correlation between pathological staging and axillary lymph node status. Conclusions There is a general lack of expression of E-CD in breast cancer; E-CD expression and milk The histological grades of tumors are closely related. The worse the differentiation, the higher the down-regulation or deletion rate of tumor cells E-CD; the adhesion between tumor cells represented by the adhesion molecule E-CD and the shedding of cancer cells in the surgical field of breast cancer. There is a relationship between E-CD deletion or down-regulation of the higher, the easier to fall off to the surgical field.
