目的调查用人乳喂养的儿童维生素D的补充率。方法基于实践研究网络进行前瞻性观察研究。网络中儿科医生需完成关于人乳喂养婴儿维生素D补充调查问卷。6~24个月儿童的父母要完成孩子启动喂养类型、人乳喂养期限、补充配方和使用多种维生素的测试。父母要说明对补充维生素D认同的程度。结果44名儿科医生做问卷调查,其中36.4%的医生表明其曾对所有人乳喂养儿童提出补充维生素D的建议。年龄相符被选儿童,共完成2 364份调查问卷;有1 140名人乳喂养婴儿至少6个月很少补充或未补充维生素;仅15.9%的婴儿补充了维生素D。维生素D的使用与父母对儿科医生建议补充(OR=7.8)、人乳含有所需维生素而无需补充(OR=0.12)的认同显著相关。主要用人乳喂养的婴儿,多数父母表示医生建议要补充维生素D,有44.6%的父母给孩子补充了维生素D。相反,有67%的父母认为人乳中含有所需维生素,其中仅3%的父母给孩子补充了维生素D。结论仅有少数人乳喂养婴儿补充了维生素D。需做好医生和父母定向教育,以增强其对维生素D补充指南的依从性。
Objective To investigate the rate of vitamin D supplementation in breastfeeding children. Methods Based on the practice research network for prospective observational studies. Pediatricians in the network are required to complete a supplementary vitamin D questionnaire on breastfeeding infants. Parents of children 6-24 months to complete the child start feeding type, duration of breastfeeding, supplementary formula and the use of multivitamins test. Parents to explain the extent of vitamin D approval. Results A total of 44 pediatricians conducted a questionnaire survey, of which 36.4% of doctors indicated that they had recommended vitamin D supplementation for all breastfeeding children. A total of 2,364 questionnaires were completed by selected age-matched children; 1,140 breast-fed infants had little or no additional vitamin for at least 6 months; and only 15.9% of infants were vitamin D supplemented. The use of vitamin D was significantly associated with a recommendation from parents to pediatricians (OR = 7.8) that human milk contains the required vitamins without supplementation (OR = 0.12). Mostly babies fed with human milk, most of the parents said the doctor suggested vitamin D supplementation and 44.6% of parents supplemented vitamin D to their children. In contrast, 67% of parents think that human milk contains the necessary vitamins, of which only 3% of parents supplemented their children with vitamin D. Conclusions Only a few breastfeeding infants are vitamin D supplemented. Good doctor and parent orientation education is required to enhance adherence to vitamin D supplementation guidelines.
