[目的]探讨胃癌患者血清中Dickkopf-1(DKK1)的表达及其临床意义。[方法]酶联免疫吸附夹心法和化学发光免疫法分别检测160例胃癌患者术前血清DKK1、CA19-9、CEA水平,以及52例胃良性疾病患者和40名正常体检者的DKK1水平,并观察其中123例患者根治术后血清DKK1变化。[结果]胃癌患者血清DKK1水平[(5.89±1.77)ng/ml]明显高于健康体检组[(3.18±1.12)ng/ml](Р<0.0001)和胃良性疾病组[(3.57±1.05)ng/ml](Р<0.0001);分层分析发现,随着疾病的进展,血清DKK1水平逐渐升高,Ⅲ期和Ⅳ期血清DKK1水平均高于Ⅰ期(Р<0.001),有淋巴结转移及肝脏转移者的血清DDK1水平升高更为明显(Р=0.006、0.028)。根治术后血清DKK1水平[(4.21±1.27)ng/ml]比术前[(5.64±1.56)ng/ml]明显降低(Р<0.001)。此外,血清DKK1水平对胃癌诊断的灵敏度和准确率均高于CA19-9和CEA。[结论]胃癌患者血清DKK1水平明显高于健康体检组和胃良性疾病组,且血清DKK1的表达水平与胃癌患者的TNM分期以及淋巴结转移、肝脏转移有关。血清DKK1表达水平可作为反映胃癌生物学行为和术后随访、监测的参考指标。
[Objective] To investigate the expression of Dickkopf-1 (DKK1) in serum of gastric cancer patients and its clinical significance. [Conclusion] The serum level of DKK1 in gastric cancer patients was significantly higher than that in healthy subjects and gastric benign disease group. The expression of DKK1 in serum was correlated with TNM stage, lymph node metastasis and liver metastasis in patients with gastric cancer. Serum DKK1 expression levels can be used as a reference indicator of gastric cancer biological behavior and postoperative follow-up, monitoring.
